By submitting this form, I confirm that I am 18 years old or above, or I am a minor with parental or guardian consent. I understand the risks associated with body piercings, including but not limited to pain, swelling, infection, or allergic reaction. I agree to follow all aftercare instructions provided by Akimi Studio to ensure proper healing. I will disclose any allergies, skin sensitivities, or medical conditions that may affect the procedure. I consent to receiving a body piercing from a trained professional using sterile, single-use needles and sanitized equipment. I acknowledge that deposits are non-refundable and appointments are by schedule only. I release Akimi Studio and its staff from any liability related to my piercing procedure and aftercare. I confirm that all information provided is accurate and that I have read and understood this consent.

Reservation Fee Policy
To confirm your appointment, a ₱500 reservation fee is required. This fee is non-refundable, as we reserve the time slot specifically for you. However, it is fully consumable and will be deducted from your total bill on the day of your appointment. Once payment is made, please send your proof of payment to us via Instagram (@akimistudio) or Facebook (Akimi Studio) to complete your booking. Please wait for a confirmation message or email before coming to the studio to ensure your appointment is secured.

Each appointment is carefully scheduled, and every minute is essential — both for your session and for the next guest. Late Arrival Policy:• A ₱100 fee will apply if you are 10 minutes late• A ₱250 fee will apply if you are 20 minutes late• Clients who arrive more than 20 minutes late may be asked to reschedule to avoid overlapping appointmentsThank you for your cooperation and for trusting Akimi Studio.
