Membership Contract and Agreement
This Agreement is made between the undersigned member (or parent/guardian) and Mecca MMA, LLC, located at 7600 75th Street, Suite 305, Kenosha, WI 53142.
Membership Term and Renewal
After the initial membership term ends, this Agreement will automatically renew on a month-to-month, 3-month, 6-month, or 12-month basis—whichever applies—at the current membership rate, which is subject to change. If I wish to change the term or cancel my membership, I agree to notify Mecca MMA in writing at least 30 days prior to my renewal date. I remain responsible for all charges incurred before cancellation. I also acknowledge that membership benefits are non-transferable without written consent from an owner of Mecca MMA.
An administrative fee of $10.00 will be charged at the time of each automatic renewal to cover processing and continuity services.
Acknowledgment by Member or Parent/Guardian
The student (or parent/guardian if under 18) acknowledges and agrees to the following:
- I voluntarily choose to participate in programs and classes at Mecca MMA.
- Completion and full payment of any program or phase is required before beginning a new program. Each new phase requires a new signed Agreement.
- Mecca MMA and its designees may photograph or film me while on the premises and use such images or footage for promotional purposes across all media platforms, including social media and websites.
- I understand the physical activity involved may be strenuous and carry risk of injury or death. I confirm that I (or my child) am physically fit to participate and have been advised to consult a physician before beginning any classes.
- In consideration of my participation, I waive all claims against Mecca MMA and its affiliates for any injury, loss, or damage arising from participation, including but not limited to death or property damage.
- I release and discharge Mecca MMA, LLC, and its representatives from any liability—including that arising from negligence or gross negligence—related to injuries, illness, or damages incurred during participation.
- I agree to indemnify and hold harmless Mecca MMA and its representatives from any claims made by third parties resulting from my actions or presence at the facility.
- I agree to follow all Mecca MMA rules and regulations.
- A $20 service fee applies to payments more than 10 days past due or any returned payments. After three such incidents, I agree to pay the remaining balance of the Agreement in full, along with any recovery costs.
- If my designated payment method fails, Mecca MMA may attempt to collect payment using any other account information I have provided, including fees outlined above.
- Mecca MMA may terminate this Agreement with 10 days’ written notice if I fail to meet my obligations and do not resolve the issue within that time.
- Non-attendance does not release me from my financial obligation for the agreed-upon membership or program.
- Mecca MMA does not offer membership breaks, freezes, or suspensions.
- I may cancel this Agreement at any time by paying a cancellation fee equal to 40% of the remaining prorated tuition (discounts do not apply). A 30-day written notice is required via email or in person to:

Rate Adjustments
Mecca MMA may adjust membership rates based on expenses, competition, or market conditions. Members will be notified at least 30 days in advance of any rate change.
Entire Agreement
This Agreement constitutes the entire understanding between the parties and supersedes all previous verbal or written agreements related to the subject matter herein.
Privacy
Mecca MMA is committed to safeguarding your personal information. We collect only what is necessary to manage your membership and communication. Questions or concerns may be directed to our Program Director.
Right to Refuse Service
Mecca MMA reserves the right to refuse service to anyone at any time, with or without a refund.
Acknowledgment and Waiver
I confirm that I have received a copy of this Agreement and understand its terms. I am of legal age (18+), or I am the parent/guardian of the participating student under 18. I have read this waiver and understand that by signing below, I am waiving certain legal rights, including the right to claim damages.
Right of Cancellation
You may cancel this Agreement within 10 days of signing by submitting written notice via registered mail, email, or personal delivery to the address above. Proof of submission within this 10-day period is sufficient regardless of when the notice is received.
