TERMS OF CONDITIONS OF CAMP ATTENDANCE
In consideration of being allowed to participate in the sports camp organized by Lightning Elite FC, I, the undersigned, on behalf of myself, my heirs, executors, administrators, and assigns, hereby acknowledge and agree to the following. En consideración a la posibilidad de participar en las campamento deportivo organizadas por Lightning Elite FC, yo, el abajo firmante, en nombre mío, de mis herederos, albaceas, administradores y cesionarios, por la presente reconozco y acepto lo siguiente:
Assumption of Risk/Asunción de Riesgo: I understand that participation in sports camp involves inherent risks, including but not limited to physical injury, illness, property damage, and the risk of contracting contagious diseases. I voluntarily and knowingly assume all such risks associated with my participation. Entiendo que participar en las campamento deportivo conlleva riesgos inherentes, incluyendo, entre otros, lesiones físicas, enfermedades, daños a la propiedad y el riesgo de contraer enfermedades contagiosas. Asumo voluntaria y conscientemente todos los riesgos asociados con mi participación.
Release and Waiver/Liberación y Renuncia: I, for myself and my heirs, hereby release, discharge, and hold harmless Lightning Elite FC, its officers, directors, coaches, volunteers, and all associated personnel, from any and all claims, demands, actions, or causes of action, whether for personal injury, property damage, or otherwise, arising out of or in connection with my participation in the sports camp, including any actions or negligence on the part of Lightning Elite FC and its personnel. Yo, en mi nombre y en el de mis herederos, por la presente libero, descargo y mantengo indemne a Lightning Elite FC, sus funcionarios, directores, entrenadores, voluntarios y todo el personal asociado, de todos y cada uno de los reclamos, demandas, acciones o causas de acción, ya sea por lesiones personales, daños a la propiedad o de otro tipo, que surjan de o en conexión con mi participación en las campamento deportivo, incluidas cualquier acción o negligencia por parte de Lightning Elite FC y su personal.
Medical Treatment Authorization/Autorización de Tratamiento Médico: I authorize Lightning Elite FC and its representatives to seek and obtain any necessary medical treatment or attention in the event of an injury, illness, or medical emergency during the sports camp. I understand that Lightning Elite FC will make reasonable efforts to contact me or my emergency contact before seeking medical treatment, but they may proceed with necessary medical care if immediate attention is required. Autorizo a Lightning Elite FC y a sus representantes a buscar y obtener cualquier tratamiento o atención médica necesaria en caso de lesión, enfermedad o emergencia médica durante las campamento deportivo. Entiendo que Lightning Elite FC hará todos los esfuerzos razonables para contactarme a mí o a mi contacto de emergencia antes de buscar tratamiento médico, pero podrán brindar la atención médica necesaria si se requiere atención inmediata.
Compliance with Rules and Guidelines/Cumplimiento de normas y directrices: I agree to comply with all rules, guidelines, and instructions provided by Lightning Elite FC during the sports camp. Acepto cumplir con todas las reglas, pautas e instrucciones proporcionadas por Lightning Elite FC durante las campamento deportivo.
Photography and Publicity Release/Autorización de fotografía y publicidad: I grant Lightning Elite FC the right to use photographs, videos, or other likenesses of me taken during the sports camp for promotional, educational, and archival purposes. Otorgo a Lightning Elite FC el derecho de usar fotografías, videos u otras imágenes mías tomadas durante las campamento deportivo con fines promocionales, educativos y de archivo.
Parent/Guardian Consent/Consentimiento de los padres/tutores: If the participant is a minor, the parent or legal guardian must consent to this waiver and release of liability on their behalf. Si el participante es menor de edad, el padre o tutor legal debe dar su consentimiento a esta renuncia y liberación de responsabilidad en su nombre.
I have read and understand this Waiver and Release of Liability, and I voluntarily sign it as my own free act and deed. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against Lightning Elite FC. He leído y comprendido esta Exención de Responsabilidad y la firmo voluntariamente.
