Jewelry Change Consent Form

By signing this document, I, the above named, acknowledge that PRACTITIONER has given me verbal discourse regarding the status of my piercing, and what will be done to complete the jewelry change and/or downsize(s) for piercings already performed.

I am allowing SHARKBITE PIERCINGS/PRACTITIONER to do what they think is reasonably necessary to install my jewelry for me safely. I understand the risks involved with the assistance of SHARKBITE PIERCINGS and that they will do what they believe is reasonably necessary to install the jewelry without damage, or to the piercing channel or me; but cannot be held liable should the item or piercing be damaged from handling, installation, removal, or sanitizing.

DOWNSIZING & JEWELRY SWAP (JEWELRY FROM STUDIO):

• Removing your piercing and re-installing jewelry to the same channel(s)
• installing new and unused fitted backings to your jewelry to help promote better healing*
• It is assumed in the case of threadless/threaded studs, the decorative end is to be returned to the prior placement and the charge only covers the applicable service fee(s) and cost of the new post(s) or jewelry.
• Some items may need to have the entire item replaced, but all existing items will be returned to the customer after the completion of the service. This will be discussed before install if applicable.
• The material(s) of the jewelry are biocompatible and generally nickel free, including ASTM-F136 Implant Grade Titanium, ASTM-F138 Implant Grade Steel, 14k Gold, 18k Gold, Niobium, Glass and Platinum.
• All jewelry sold is assumed to be free of defects.
• All jewelry is sold by piece/part; it is STANDARD to reieve an implant grade titanium post/backing even with an upgraded purchase of 14k/18k GOLD.
• If there is any known allergy, please alert us.
• No refunds will be given for jewelry lost. Once you leave the studio, it is the CLIENT responsibility to care for the jewelry and piercing.
• Applicable jewelry warranties do not cover loss of goods.

JEWELRY SWAP (JEWELRY NOT FROM STUDIO)

• For jewelry provided by the client, client assumes any and all responsibility in the event they are allergic or have an adverse reaction to the jewelry provided. PRACTIONER is not liable for any damages as a result of the insertion.
• Any jewelry that is not brand new cannot be re-sterilized. Items will be sanitized with a soak of Opticide or Maticide, wiped with an alcohol pad and installed.
• CLIENT assumes the risk due to possibility of damage to the jewelry from sanitizing. PRACTIONER will only do what is reasonably necessary to perform the requested service, but cannot be held liable in the event there is damage from anything performed regarding this service. PRACTIONER will inform CLIENT should any problems be visible. Risk is minimal, but not zero.
• If I am unsure of the material, I understand the piercer cannot reasonably know if any allergy may arise, or if the procedures they use to sterilize jewelry may possibly damage my item irreparably.
• Due to not knowing the materials, the jewelry and/or piercing channel may be permanently damaged or lost as a result of the installed item(s).
• PRACTIONER AND STUDIO are not liable for loss due to allergy or jewelry failure or loss.

I hold the practitioner forever harmless should any issue arise from the swap of the jewelry. Should any questions or concerns arise I will contact the practitioner at my earliest convenience so they can assist best of their knowledge.
