I hereby appoint Acuity Business Advisory as my Accountants, Tax Agent, ASIC Agent, and SMSF Auditor to collect information on my behalf, conduct interviews, complete, and lodge my tax return. Additionally, I appoint Acuity Business Advisory as my Tax Agents to liaise with the ATO on my behalf for current, past, and future tax matters related to me or any entities I am associated with. This includes all matters related to Tax, GST, FBT, ABN, or any other queries, whether verbal or written, until this authority is specifically revoked by me in writing.

I further declare that I agree with the various incomes, deductions, and offsets claimed, and the information provided by me is true and correct to the best of my knowledge and belief. I declare that where receipts are not available, the expenses were genuinely incurred by me and were directly relevant to my employment or business. I am aware that I shall be liable for any incorrect information provided by me to Acuity Business Advisory and its associates or to the ATO.

I also authorize Acuity Business Advisory to collect their fee from any tax refund if I have any outstanding and unpaid invoices. By this declaration, I authorize the appointed agents to perform duties as delegated from time to time, including the lodgment of my tax return based solely on this electronic declaration provided by me.
