  • COURSE PROVISIONS

  • Thumb CDL, LLC will provide a minimum of ____ hours of in person classroom instruction, ____ hours of range skills, ____ hours of driving observation, and ____
    hours of public road instruction for CDL Training. This course is Entry Level Driver Training (ELDT) compliant and you must score an 80% or higher to pass. Thumb
    CDL,LLC will conduct the behind-the-wheel instruction in a tractor-trailer that is fully insured to cover each student enrolled in the program. Students must obtain
    a Commercial License Permit (CLP) by achieving a score of 80% or higher on the State of Michigan “Knowledge” test. Qualified applicants may receive assistance
    from a funding institution (i.e. Michigan Works, ISD for ISD students, employer) for up to course cost. Any balance after assistance must be paid in full, by the student,
    before the 3rd party test can be taken. Classes will normally run during school hours for ISD students (if school is cancelled due to weather, classroom/drives will
    also be cancelled) for_____ weeks for a total of ____ hours.

  • TERMS

  • 1. Prior to beginning the class, Thumb CDL,LLC must have received authorization from the funding institution.
    2. Students must present a current US DOT physical examination card and drug screen at the time the course begins.
    3. Students must be at least 17 years old to take the classroom portion of this training. Students must be 18 years old to
    obtain their CLP from the Secretary of State for the behind the wheel portion of this training.
    4. Total fee for this course: _____________.
    5. Payment methods accepted: credit card (3.55% additional convenience fee will be added to the total amount when using a
    credit card), cash, money order, or checks. Thumb CDL, LLC will not accept postdated checks. Payment plans are available,
    if needed, and must be pre-approved prior to the beginning of class.
    6. Student cellphone use is not permitted during any instruction (classroom or behind the wheel). Failure to meet this
    expectation may result in dismissal of the course.
    7. Student attendance is required for all class times. In the event of an emergency, our instructor(s) need to be notified as
    soon as possible. Missed in-person/hybrid classroom sessions must be made up on the student’s time
    (evenings/weekends). Students will be given materials and a quiz that must be completed with an 80% or higher.
    8. There is no charge for any materials or supplies provided during the course, however, students will be responsible for the
    following fees: DOT physical (made payable to the Medical Examiner), DOT Drug Screening (made payable to Thumb CDL,
    LLC), Commercial Learner’s Permit (CLP) made payable to the State of Michigan Secretary of State, and All Road Test Fees
    (made payable to the tester).
    9. The use of Thumb CDL, LLC’s tractor-trailer for 3rd party testing is included in tuition cost. However, if a student fails the
    road test, they will be responsible for the additional road testing fee as well as truck rental. Truck rental may be split
    amongst multiple students if applicable. Financial help may be available for these costs if needed.
    10. By signing this contract, the student agrees to not consume alcohol and/or controlled substances (without prescription) for
    24 hours prior to getting behind the wheel of our Commercial Motor Vehicles (CMVs).

  • 11. Students are expected to complete the course in the allotted amount of time for that specific course (ex. CDL A 160 would
    be 160 hours completed). If the student cannot complete by the end of the course, they will have the option to purchase
    more time at a rate of $125.00/hour up to 10 hours. If the student chooses not to purchase extra time or cannot complete
    within the extra time they have purchased, they will be removed from the program.
    12. If a student has completed all requirements for ELDT training and requests their 3rd party skills test but still has remaining
    hours in road or range work, they will be required to sign our test out form. They will not receive a refund for the
    remaining hours paid due to Thumb CDL, LLC’s courses being offered at a discounted rate compared to the hourly rate. If
    the student is unable to pass their 3rd party skills test, they will return to training and complete their remaining hours.
    13. If a student misses any scheduled appointment—including classroom, range, behind-the-wheel instruction, or third-party
    testing—without 24 hours prior notice or a valid reason (determined by our office), a $100.00 fee will apply. This fee must
    be paid in full before the student’s third-party test is scheduled. If the student has missed more than 3 appointments, they
    may be removed from the course. Note: The skills testing entity has the right to charge their own fee for a cancelled tests
    and will accept their own payment. Thumb CDL, LLC is not responsible for payments to the testing entity on the student’s
    behalf.
    14. Students are required to schedule at least 2 behind the week sessions a week. Failure to meet this requirement may result
    in removal from the course. If the student is having difficulty scheduling due to no available appointments, they must
    contact our office. If there is no scheduled appointment and no contact with our office, we will issue a letter via email to
    the email address the student has provided in this contract requesting their communication and cooperation with
    scheduling. No communication from the student after the email is sent may result in dismissal from the course.
    15. For Self-Pay Students: Half of the cost of the course is payable at the first classroom instruction date. The full course cost
    must be paid in full before the 3rd party skills test is taken. Payment plans must be reviewed and approved prior to the first
    day of class.
    16. For Michigan Works Students: Students are responsible to follow up with their funding institution to receive qualification
    for funding. If the student doesn’t follow through with funding approval, they are responsible for the full amount of the
    course.
    17. The student consents to Thumb CDL, LLC using their picture and/or video in social media postings and any promotional
    materials. If the student DOES NOT consent, they may request a “Do Not Consent” form and no pictures or video will be
    used.

    18. SMS/Text Messaging Consent (A2P 10DLC Compliance): I understand and agree that Thumb CDL, LLC may send me training-
    related notifications via SMS/text messaging using an automated system. These messages may include schedule changes,

    reminders, training updates, or other time-sensitive information. Message and data rates may apply. I may opt out at any
    time by replying “STOP.” Consent is not a condition of enrollment.
    I, the student, agree to receive SMS/text messages from Thumb CDL, LLC for training and communication purposes.

  • REQUIREMENTS TO PASS THE COURSE

  • 1. An 80% minimum score is required to pass this course (before moving onto the 3rd party state examination

    2.It is the student's responsibility to learn the curriculum provided. All homework, quizzes, and assessment tests

    must be completed to pass this course. Thumb CDL, LLC does not guarantee that each student will pass this class.

    NO REFUNDS will be offered after day 2 of class.

    condition has been corrected and/or is under control and the student meets the physical and mental requirements to operate a commercial motor vehicle as that term is defined in section 7a of the Michigan Vehicle Code, 1949 PA 300, MCL 257.7a.
