Hair Extension Application Form

Hello Beautiful! Thank you for your interest in becoming a client at Hue & Mane. Because I take a limited number of new extensions clients each month, I ask every prospective guest to complete a brief consultation survey. The information you provide ensures that your experience is personalized from the very beginning.

A $75.00 deposit is required to secure your consultation reservation. This deposit is fully refundable if you cancel or reschedule at least 48 hours before your scheduled appointment time. Cancellations made within 48 hours of the appointment forfeit the deposit. If you move forward with your hair extensions, your deposit will be applied toward your total investment.

After you submit this application, I personally review every response. If it looks like we're a great fit, I'll follow up within 24 business hours with available consultation times and a link to secure your spot with your deposit. If I don't think we're the right match yet, I'll let you know that too, along with next steps.

The investment in extensions generally ranges from $1,100–$2,500, total amount will be finalized after your consultation based on your goals.

All extension methods require maintenance appointments at times to be determined, typically every 6–12 weeks and generally range from $150–$400+ depending on method. (Exact maintenance requirements depend partly on method chosen and amount of hair required to complete the desired look). Are you able to commit to this ongoing schedule?
