Swings4Theo 2nd Annual Wiffle Ball Registration

Join us on Saturday, September 20, 2025 at Georgetown Little League in Fort Wayne, IN for the 2nd annual Wiffle Ball Games. All registrations are individual and teams will be created at the event. This is a day designed to focus on having fun, a day where the little kids can play with the big kids (old kids are welcome, too)! Sign up to join us and be a part of this special event.

Liability Release and Waiver of Claims

In consideration of the opportunity for my child to participate in the Swings4Theo Annual Wiffle Ball Game (the “Event”), I, the undersigned, being the parent or legal guardian of the participant, hereby agree to the following on behalf of myself and my child:

I acknowledge and understand that participation in the Event involves physical activities with inherent risks that could result in injury or death. I accept full responsibility for any such risks, whether known or unknown, even if arising from the negligence of the Released Parties (as defined below).

To the fullest extent permitted by Indiana law, I hereby release, waive, discharge, and covenant not to sue Swings4Theo, their directors, officers, volunteers, employees, agents, affiliates, sponsors, and all other associated personnel or entities (collectively, the “Released Parties”) from any and all claims, demands, actions, or causes of action for injury, loss, damage or death, including but not limited to bodily injury, personal injury, property damage, or wrongful death arising out of or related to participation in the Event whether or not caused by the negligence of the Released Parties.

I agree to indemnify and hold harmless the Released Parties from any and all liabilities, claims, or expenses (including attorneys’ fees) resulting from any claim arising out of my child’s participation in the Event.

I authorize the Event organizers or their representatives to administer basic first aid and, if necessary, seek emergency medical treatment on my child’s behalf. I understand that I am responsible for any medical costs incurred.

I understand that all participants are expected to behave respectfully, follow all rules and safety instructions, and act in a manner that promotes a positive experience. I will ensure my child complies with these expectations.

I hereby grant permission to Swings4theo or their volunteer photographers to use my image, voice, and likeness in photographs, videos, and other digital media on their social media platforms, including but not limited to Facebook, Instagram, Twitter. Swings4Theo is authorized to use this media for promotional and marketing purposes related to their organization. I understand that I will not receive any compensation for this use. I waive any rights of compensation or ownership to the media created. By signing below, I acknowledge that I have read and understand the terms of this release. I understand I may opt out of this provision by providing written notice before the Event.

This agreement shall be governed by the laws of the State of Indiana. Any disputes arising out of this agreement shall be resolved in a court of competent jurisdiction located in the state of Indiana.

I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY. I SIGN IT VOLUNTARILY WITH THE INTENT TO RELEASE THE RELEASED PARTIES FROM LIABILITY TO THE MAXIMUM EXTENT PERMITTED BY INDIANA LAW.
