By signing this form, I acknowledge and agree to the following:

Assumption of Risk:
I understand that participation in training, workouts, and related activities at El Norte Performance involves inherent risks, including but not limited to injury, illness, or death. I voluntarily assume all such risks.

Release of Liability:
I release and discharge El Norte Performance, its owners, staff, trainers, and affiliates from any and all liability, claims, demands, or causes of action that may arise from my participation, including negligence.

Medical Authorization:
In the event of a medical emergency, I authorize El Norte Performance staff to seek medical care on my behalf. I understand I am responsible for any resulting costs.

Photo/Video Release:
I consent to the use of photographs or videos taken during training sessions for marketing or promotional purposes.

Minors:
If the participant is under 18, I certify that I am their legal parent or guardian and consent to this agreement on their behalf.
