By signing below, I agree to the team entry and/or sponsorship level selected above. I understand that sponsorship levels are available on a first-come, first-served basis.

I acknowledge that no refunds will be issued should a team withdraw from the tournament, as this is a fundraising event. In the event of inclement weather, a rescheduled date will be announced. If a team is unable to participate on the make-up date due to weather-related changes, a refund will be issued.

I understand that participation in this event is at my own risk. I agree to hold harmless the Mineola Chamber of Commerce, the City of Mineola, and the Mineola Country Club from any and all liability associated with this event.
