PARTICIPANT WAIVER & AGREEMENT

Please read the waiver & agreement. The waiver & agreement becomes applicable if chosen to perform in the Annual Showcase.

PLEASE NOTE: All performers aged 16 years and older are required to sign the Performance Submission Form. A parent/guardian's signature is also required for all performers UNDER the age of 18 Years Old.
By signing & submitting the Performance Submission and Partcipant Agreement, you are certifying that you have read and fully accept and understand the terms and conditions of this Performance Submission and Partcipant Agreement. And that you understand that becoming a performer, if chosen, creates a binding agreement to these terms and conditions. In addition, you are certifying that all information is correct and that you are the person completing this Performance Submission and Participant Agreement for you or as the parent/guardian. When you press the submit button, an email copy of the Performer Submission and Participant Agreement will be sent to you.
