This form is to be completed by students and parents/guardians who are requesting to miss a scheduled band rehearsal, performance, sectional, or other events. Requests must be submitted at least 3 weeks in advance.Submission does not guarantee approval, all requests will be reviewed by the director. For emergencies (e.g., illness, family crisis), please also email Mr. Hernandez directly at brhernandez@avhsd.org.
