The purpose and intent of this official document is to establish a binding agreement between a junior volleyball player and Jammers Volleyball Club for the duration of the current USAV Indoor competitive season (the indoor season ends at the 2026 USAV Junior National Championships regardless of whether a team/player/club continues to play through that event). Signed commitment letters will be kept on file by Jammers Volleyball Club and serve as official documentation of affiliation. 

Player - By signing this Commitment Form I understand and agree to the following:

Parent/Guardian Signature Required. A parent or legal guardian is required to sign this Commitment Form if the student-athlete is less than 18 years of age at the time of signing.

Commitment. I understand that once I sign and submit this document to a club representative that I shall not attend any other club's tryouts. If any other clubs contact me about participating with their club during the USAV season, I may choose to notify them that I have signed a "Commitment Form" and have committed to Jammers Volleyball Club for the 2025-2026 indoor season (through July 4, 2026). 

Binding Agreement. I understand that I have signed this Commitment Form with the Jammers Volleyball Club and not with a particular individual. If a coach or any player(s) leave(s) the team, I remain bound to the club by the provisions of this document.

Nullification of Other Agreements. My signature on this Commitment Form nullifies any agreements, verbal or otherwise, which would release me from the condition stated within this document.

 

By signing this Commitment Form, I acknowledge the following penalties should I break this agreement:

Basic penalty: I understand that by signing this letter I am committed to joining the Jammers Volleyball Club for the entire 2025-2026 season. If I compete with another club, I will be subject to suspension for the remainder of the season and still bound to the terms of the Financial Responsibility Agreement.

Certification: By my signature, I certify that I have read all terms and conditions in this document. I fully understand, accept, and agree to be bound by them.

This agreement contains the details and family commitments outlined, in its entirety, on the JVC website (www.jammersvolleyball.com), for the Jammers Volleyball Club (JVC) members. The basis of the dues includes, but is not limited to, tournament registration costs, coach’s pay and expenses, gym costs, and JVC equipment and operation costs. The purpose of this agreement is to ensure that all JVC members and their parents have read and understood the financial commitments and responsibilities, are familiar with the financial commitments required to participate with JVC, and that their respective signature(s) below affirms this understanding and promise to fulfill ALL necessary financial commitments to support their respective JVC team during the entire Club season.

Dues are paid to the club as a whole and not to individual team accounts. JVC does not give dues refunds for any reason, including a team or a family decision not to attend a budgeted tournament. Teams can apply the cost of an unattended tournament towards another if done within the deadlines of both tournaments.

I hereby affirm, through my signature below, that I understand the cost of the Jammers Volleyball Club (JVC) dues and how the dues are outlined on the JVC website (www.jammersvolleyball.com).  I understand and agree, through my signature below, that the dues cover practices, tournament registration costs, coach’s pay, and expenses for the months of August 2025 through May 2026.

Additional Tournaments

If any team that my daughter plays for, through a majority vote that can field at least 6 players, elects to participate in any tournament above and beyond the tournaments set in the Scheduled Tournaments, then I agree to the following: If I voted in favor of my daughter participating in the tournament, I will support the team by having my daughter participate in the tournament. If I voted in favor of my daughter participating in the tournament, I will financially support the team by paying the pro-rated per player amount for coaches’ tournament pay, coaches’ expenses and tournament registration.  Payment shall be submitted to JVC prior to registration for the event that causes the sum to be exceeded and immediately following a request for payment.

USAV Junior Nationals

If my daughter plays for a Jammers VBC team that qualifies for a National Bid to the USAV Junior National Championship event normally held in June/July, I agree to the following:

• To attend the USAV Junior Nationals event in June/July.
• I agree to support the team by having my daughter participate in the USAV Nationals tournament.
• I agree to financially support the team by paying the pro-rated per player coaches pay, for the months of May, June and possibly July up to and including the Nationals tournament.

I agree to financially support the team by paying the pro-rated per player coaches’ expenses and USAV Nationals Tournament registration. I agree to this provision as a commitment to accepting an invitation for my daughters’ participation on a JVC team. I also understand and agree that the JVC Executive board will impose consequences that could adversely affect my family’s participation with JVC in the event the obligation I have agreed to herein is not fulfilled.  I agree to indemnify and hold harmless Jammers Volleyball Club, its partners, officers, employees, and any organization co-sponsoring the program, from and against any and all liability arising out of or in any way connected with the dues, additional tournament costs, coach’s pay and/or coaches expenses and travel.

My primary goal is to become the very best player I can be. I am responsible for my own actions. I am fully committed to the Jammers Volleyball Club and therefore will:

• Conduct myself in a manner that is respectful to myself, to my team and to my club.
• Budget my time to meet all the responsibilities of work, school and other personal areas.
• Attend every possible practice, tournament and team function. If I am unable to attend, I will personally notify my coach ahead of time.
• Be ready for practice on time (15 minutes early), arrive on time, be ready on time.
• Pursue mental and physical conditioning outside of practice.
• Communicate with my teammates and be a part of the team.
• Discipline my body, including getting adequate sleep, maintaining a healthy diet and abstaining from alcohol, tobacco, vaping and illegal drugs.
• Learn from my mistakes and never make excuses or lay blame.
• Accept all coaching comments and assignments as a way in which the team and myself might improve.
• Ignore the errors of my teammates, as I believe no one is intentionally trying to make a mistake.
• Support the full effort and good skills that my teammates exhibit.
• Perform with full effort and intensity to the best of my ability in all practices and tournaments.
• Concentrate, focus and avoid causing distractions, which interfere with my team’s performance.
• Clean our tournament bench/areas and practice facilities of all trash. Leave the area as it was found or better.
• Realize that a team is made up of individuals and everyone cannot start. Accept my role on the team and what it takes to be the best.

Signing of this contract acknowledges that I have read and agree to abide by all of the stipulations listed on this document.

One of my goals with Jammers Volleyball Club is to help my child become the best volleyball player and person they can be. I understand that participation in the club requires I conduct myself with these Jammers Volleyball Club Parent Commandments at the forefront of my actions and behaviors. I understand that a failure to meet these expectations will mean that I will not be permitted to attend practices or tournaments.

ATTENDANCE: I will help my child attend all practice sessions, matches and tournaments. If my child is unable to attend practice or a training session, I will notify the Coach or Assistant Coach at least 1 day prior to the session to be missed. If my child is unable to attend a tournament, I will notify the Coach or Assistant Coach at least 1 week prior to the tournament to be missed.

SPORTMANSHIP: I will always be a positive example to my child in the area of good sportsmanship during practices and tournament. I will refrain from criticizing officials, opposing players, other parents, coaches, my child’s teammates and my child. I will exhibit exemplary sportsmanship whether our team is winning or losing, playing well or not playing well. I will be a positive role model to my child, the other players, their parents and our coaches.

HELPFULNESS: As a parent, I will assist with gym or team cleanup and encourage my child to share all referee, line judge and work responsibilities.

COOPERATION: I will cooperate with my child’s coaches, Jammers Administration and with the Club Director regarding Team issues and financial commitments or concerns regarding the club.

APPEARANCE: I will help ensure my child is appropriately dressed for all practices and tournaments both in and outside of the gym. This includes helping to ensure that my child wears cover shorts when leaving the gym or playing venue.

COMMITMENT: I will regularly check my email and team group chats, recognizing that these electronic methods are the means by which Jammers will inform me of practice changes, tournament schedules, hotel information and any other pertinent information. I will make sure that my child has adequate, safe and on-time transportation to and from all practices and tournaments.

COMMUNICATION: I will allow my child to work to resolve problems or issues on the team with the coaching staff. I will NEVER confront coaches at a tournament or practice about playing time or coaching techniques. If I absolutely feel the need to discuss this type of problem with my child’s coach, I will ask to schedule a meeting involving one of our club administrators and the coach.

ATTITUDE: I will encourage my child to work hard at practices and tournaments. I will encourage my child to take the time with the team seriously. I will ensure my child is attentive and focused on all coaching instruction.

PAYMENT: I will make timely payments of Dues to the Club for my child’s participation with Jammers Volleyball Club. I understand and agree to the financial commitments of the club.

Signing of this contract acknowledges that I have read, understand, and agree to abide by all the stipulations listed above.

• I will refrain from using any social media source to post, send or otherwise deliver any remarks or comments that could be determined as detrimental to Jammers Volleyball Club, its staff or any of its memebers. 
• I will never encourage anyone to post, send or otherwise deliver any remarks or comments that could be determined as detrimental to Jammers Volleyball Club, its staff or any of its members.
• I will report or inform the Jammers Volleyball Club Administration of any posting, sending or delivery of any remarks or comments that could be detrimental to Jammers Volleyball Club, its staff or any of its members.
• I understand and agree that I will be subject to disciplinary action if it is determined that I engaged in or encouraged any posting, sending or delivery of any remarks or comments in social media that could be detrimental to Jammers Volleyball Club, its staff or any of its members.
• I understand and agree that the disciplinary action taken can be, but not limited to, a meeting with club administrators and all parties involved, limited suspension from club or team activities; including practices and or tournaments, dismissal from the team and club, and possible reporting to appropriate law enforcement.

By signing this agreement below, I acknowledge that I have read, understand, and agree to abide by the Jammers Volleyball Club No Tolerance Social Media Agreement as outlined above.

I acknowledge that volleyball or any sporting event is an extreme test of a person’s physical and mental limits and that my participation in a volleyball event can cause potential death, serious injury, or property damage. With a full understanding of the potential risks, I HEREBY ASSUME THE RISKS OF PARTICIPATING OR OFFICIATING IN A VOLLEYBALL EVENT.

I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors and assigns:

1. I WAIVE, RELEASE, AND DISCHARGE from any and all claims or liabilities for death or personal injury or damages of any kind, EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIGENCE AND/OR WANTON MISCONDUCT OF PERSONS OR ENTITIES LISTED BELOW, which arise out of or relate to my traveling to and from or my participation in any volleyball event, THE FOLLOWING PERSONS OR ENTITIES: USA Volleyball and its Regional Volleyball Associations, tournament directors, sponsors, and the officers, directors, employees, representatives, and agents of any of the above;
2. I AGREE NOT TO SUE any of the persons or entities listed above for any of the claims or liabilities that I have waived, released or discharged herein;
3. I INDEMNIFY AND HOLD HARMLESS the persons or entities mentioned above from any claims made or liabilities assessed against them as a result of my actions.

COVID-19 Waiver: Jammers Volleyball Club (JVC) has put numerous preventative measures in place to reduce the likelihood of spreading COVID-19, however, JVC cannot guarantee that you or your child(ren) will not become infected with COVID-19.  I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, JVC Directors, Coaches, Athletes and their families. I acknowledge and voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren), my family or myself including, but not limited to, personal injury, disability, illness, death, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s presence or participation at JVC activities.  On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, or hold harmless JVC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.  By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily agree to assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending and participating in JVC activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of JVC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any JVC activity.

 

Agreement: I/We have read, understand and agree to comply with the USA Volleyball Waiver and Release of Liability as outlined above.

If applicant is under 18 years of age, a parent or guardian must execute, in addition to the foregoing Waiver and Release, the following, for and on behalf of the minor. The undersigned parent and natural guardian or legal guardian of the applicant (being registered) executes the foregoing Waiver and Release for and on behalf of the minor named herein. I hereby bind myself, the minor and all other assigns to the terms of the Waiver and Release. I represent that I have legal capacity and authority to act for and on behalf of the minor named herein, and I agree to indemnify and hold harmless the persons or entities named in the Waiver and Release for any claims or liabilities assessed against them as a result of any insufficiency of my legal capacity or authority to act for and on behalf of the minor in the execution of the Waiver and Release. I fully consent to my child’s participation in USAV/RVA events.

USA Volleyball (“USAV”) and its Regional Volleyball Associations (“Regions”) encourage participation and the pursuit of excellence in all facets of volleyball.
USAV and the Regions grant the privilege of membership to individuals committed to this same pursuit. The privilege of membership may, therefore, be withdrawn or denied by USAV and/or the Regions at any time where USAV and/or the Regions determine that a member or prospective member’s conduct is contrary to the best interests of the sport and those who participate in it.

In order to assist all members in serving the best interests of volleyball and to create an environment that is supportive, inclusive, positive and appropriate, USA Volleyball and its Regions have adopted this Code of Conduct. As a condition of membership, each prospective member is required to acknowledge and adhere to this Code of Conduct.

THE FOLLOWING SHALL BE CONSIDERED VIOLATIONS OF THE USA VOLLEYBALL CODE OF CONDUCT:

1. Violation of any anti-doping policies, protocols or procedures as defined by the International Olympic Committee (IOC; www.olympic.org), World AntiDoping Agency (WADA; www.wada-ama.org), Federation Internationale de Volleyball (FIVB; www.fivb.org), US Anti-Doping Agency (USADA; www.usada.org) or the United States Olympic Committee (USOC; www.teamusa.org). Violations of this provision will be adjudicated only by USADA or the proper anti-doping authority, not USAV.
2. Violation of SafeSport rules, policies and procedures promulgated by the U.S. Center for SafeSport (www.safesport.org), including the SafeSport Code and/or Minor Athlete Abuse Prevention Policies (“MAAPP”), as they may be amended from time to time.
3. Discrimination in violation of the Amateur Sports Act which requires that USAV must provide an equal opportunity to athletes, coaches, trainers, managers, administrators, and officials to participate in the sport of volleyball. Members must be allowed to participate and compete to the fullest extent allowed by USAV and Region rules. Discrimination against any member on the basis of age, gender, race, ethnicity, culture, religion, sexual orientation, gender expression, gender identity, genetics, mental or physical disability, or any other status protected by federal, state or local law, where applicable, is prohibited.
4. Possession, consumption, sale or distribution of alcohol, tobacco, vaping devices or e-cigarettes, marijuana, illegal drugs, or controlled substances, if
   such possession, consumption, or distribution is illegal or in violation of USAV or Region policy.
5. USAV policy prohibits the possession, consumption, sale or distribution of alcohol, tobacco, vaping devices or e-cigarettes by anyone registered as a
   junior volleyball player at the event venue of any USAV/Region sanctioned junior event.
6. Use of a recognized identification card by anyone other than the individual described on the card.
7. Physical damage to a facility or theft of items from a room, dormitory, residence or other person. (Restitution will be part of any penalty imposed.)
8. Possession of fireworks, ammunition, firearms, or other weapons or any item or material which by commonly accepted practices and principles would
   be a hazard or harmful to other persons at USAV/RVA sanctioned events.
9. Any action considered to be an offense under Federal, State or local law ordinances.
10. Violation of the specific policies, regulations, and/or procedures of the USAV, Region or the facility used in conjunction with a sanctioned event. (It is the
    responsibility of the individual to be familiar with applicable specific policies, regulations and procedures.)
11. Conduct which is inappropriate as determined by comparison to normally accepted behavior.
12. Physical or verbal intimidation of any individual.
13. Any act of fraud, deception or dishonesty in connection with any USAVrelated activity.
14. Actions that will be detrimental to USAV or the Region.
15. Any non-consensual physical contact, obscene language or gesture, or other threatening language or conduct directed towards any personnel associated with a USAV or Region sanctioned event including but not limited to event staff, match officials, arbitrators, ticketing personnel, etc.
16. To act in such a manner, either through behavior or policies, as to threaten an athlete’s amateur status or Region, USAV, school and/or collegiate eligibility.
17. To not abide by applicable USAV and/or region transfer policies.
18. To not abide by applicable Region recruiting policies.

• I have read and understand the USA Volleyball Code of Conduct.
• I agree and consent to abide by the USA Volleyball Code of Conduct and Disciplinary Policies and other region specific code of conducts.
• I understand that, if I violate the USAV and/or RVA Codes of Conduct, I might be subject to disciplinary action in accordance with USAV and/or
  RVA.
