Souler Float is a serene and peaceful wellness center that requires guests maintain responsible and respectful conduct at all times.

We kindly ask you, your guests and accompanied children respect the Zen environment throughout your visit so that all clients may enjoy their time at the center.

PLEASE ACKNOWLEDGE ALL OF THE FOLLOWING STATEMENTS TO ENSURE YOUR TIME AT SOULER FLOAT WELLNESS CENTER IS SAFE, RELAXING, AND ENJOYABLE.

• Please leave your shoes at reception and wear our spa slippers during your visit.
• Please do not photograph other guests and ask permission to take photos of yourself or
members of your party during your visit
• Please refrain from applying/wearing perfumes, lotions, oils, fragrances, etc.
• It is recommended to wear comfortable clothing for your visit.
• No glass items, food or beverages in the therapy rooms
• No animals in the therapy rooms (except task trained service animals)
• During pregnancy, please consult with your doctor before embarking on any therapy
• No nudity or horseplay anywhere at the property
• No EMT or medical practitioner available on site, if you become unwell during your visit an
ambulance will be called
• Children should be supervised at all times
• All visitors should observe Spa and Wellness facility etiquette during their visit
• All new clients or existing clients embarking on a new wellness program at Souler Float Wellness Center, must arrive 15minutes prior to my scheduled appointment time to allow for assessment and orientation.

We reserve the right to cancel any appointment where the information provided on this form deems the patron unsuitable for the selected therapy or whereby consultation with a qualified physician is required before proceeding.

Pregnant women, small children and people with health issues that cause drowsiness should not use any of the therapies without first consulting a doctor and obtaining a note from him/her before your visit to Souler Float.

If you arrive at the center under the influence of alcohol, narcotics, other mind altering or relaxants you will be refused service.

You must have good hygiene and be wearing suitable attire for your visit. No swim, beach wear or shirtless is permitted in the public areas.

Payment
Please be advised that we are a cashless center, we accept Mastercard, Visa, American Express and Discover.

Some HSA/FSA Account types offer a payment card feature for wellness therapies, where this is provided we can accept as payment however please be aware not all HSA/FSA providers offer this feature so we cannot guarantee the payment will be successful. It is recommended that you bring a backup payment with you for your session.

Only in exceptional circumstances will we accept cash. We DO NOT have a cash till on site so please have exact payment amount, please be aware we may not be able to provide you with change. If change is required, please let us know you would like to pay in cash prior to your session, if change is not immediately available it can be collected at a later date.

ALL SERVICES ARE PRE-PAID, PAYMENT IS EXPECTED IN ADVANCE OR AT TIME OF BOOKING - ALL SALES ARE FINAL.

Oils, lotions and fragrances can damage the sauna booth surface. I confirm NOT to have applied, nor will I apply any of these whilst in the sauna or sauna suite. Should I use any of these products resulting in stains and damage to the sauna booth, I will pay a repair/restoration and lost business fee, minimum $500 payable on the day of incident.**

It is always important to maintain proper hydration levels during Full Spectrum Infra-Red Therapy. Dehydration will increase carbohydrate utilization and cause less fat to be burned for energy. We highly recommend drinking a minimum of 8oz of water prior to entering the sauna and a minimum of 8oz of water after sauna use.*

Important Safeguards:

Never sleep inside the sauna while it is on.

Souler Float's sauna has heat elements in all walls and floor, do not place a towel or any clothes on top of the sauna floor while turned on.

Water, other Liquids, Plastic, glass bottles or cups should be left outside the sauna during the session.

Important Safeguards:

I will turn off all of my electronic devices prior to the start of my session.

· I agree that any cologne or perfume will be removed from my body prior to starting my Dry Float Session.

· I agree that if I smoke, I will refrain from doing so for at least 45 minutes prior to my dry float session to avoid bringing the odor of smoke into the treatment room.

· I do not have any communicable or infectious disease, illness or skin disorders.

· I do not suffer from uncontrolled seizures or epilepsy.

· I am physically capable of getting up and down onto the dry float system on my own. If unable, I agree that I will arrive with a certified aide to help me in and out of my session.

Dry Floatation therapy provides a deep state of relaxation that stimulates blood flow through all of the body’s tissues, reduces stress levels and releases natural endorphins. To ensure a comfortable, clean and safe floatation experience, I agree to the following (please initial below if you agree to the following statements):

· I will not have anything in my pockets during my dry float session.

· I will remove my shoes and any clothing or accessory items (such as jewellery, hair clips or belt) with sharp edges which may damage the membrane prior to my dry float session.

Waiver of Liability, Release and Hold Harmless Agreement:

In consideration for using the Souler Float Wellness Facility and Services I hereby RELEASE, WAIVE, DISCHARGE, and HOLD HARMLESS Souler Float Wellness Center, its operating LLC, owner and employees from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by any person, while using the equipment or due to the use of the equipment at the Souler Float Wellness Center.

I hereby acknowledge, agree, irrovoably and voluntarily assume the risk of injury, accident or death which may arise whilst at the Souler Float Wellness Center and use of its services. I, and any of my heirs, executors, representatives, or assignee hereby release all claims or liabilities which prevents me from filing suit or making claims for personal injury or property damages of any kind sustained while on the premises, during a session and from any advice provided by an employee, independent contractor or any representative.

I agree that this Waiver is in effect for all services and will not expire unless specifically requested by either party.

If it is found that any of the preceding points were agreed to untruthfully and /or were not adhered to, we reserve the right to cancel an appointment without refund or to request the guest reschedule.

I, a client of Souler Float, hereby release Souler Float and its directors, officers, employees, agents and professional staff from all actions, causes of actions, suits, claims, liability, damages and demands of any kind, whether direct, indirect, special, exemplary or consequential, including interest therein of Souler Float Wellness Center which may occur as a result of any injury including death sustained by myself or others resulting from the receipt of any wellness therapies.

· I will take extra precautions for my own safety. I assume any and all liability whatsoever arising out of or related to any loss, injury and or damage sustained by any persion, whilst using the facilities, services or equipment resulting from any slip and fall incident.
Guest or Authorized Representative : By signing this waiver, I have read and fully understand all statements in its entirety and agree to the terms of this liability waiver agreement. I confirm not have any of the listed contraindications or have consulted with a medical practitioner, agree to the terms and conditions of use and accept all risks voluntarily. I understand this is a release of liability, which prevents me from filing suit or making claims for damages. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of Florida.*
