CONTEST DETAILS

We encourage you to share up to 3 original photos that capture:

1. Diversity: Celebrate our community by showcasing the unique backgrounds and cultures from your home country or neighborhood.

2. See Yourself: Share images that reflect the feeling of truly being represented, seen, and valued, or moments where you recognize yourself and your story.

3. Community in Space: Capture how spaces bring people together, fostering a sense of connection and a shared sense of belonging.

SELECTION CRITERIA

We are looking for photos that tell your story. Whether it’s what you call home, or the people you call family, we want to see and represent your community at Sunridge Mall Clinic.

• Relevance to Theme: Photos that strongly represent the themes of diversity, community, and a welcoming space.

• Creativity and Originality: Unique and engaging perspectives that resonate with others.

• Visual Appeal: High-quality, well-composed images that are suitable for display in our clinic.

Selected photo submissions will be featured in a special exhibit space within the clinic, as well as shared on our website and social channels.

We can’t wait to see how you interpret these themes and how your photos can help make our space feel even more welcoming and inclusive!

OR send your photos to communications@mosaicpcn.ca and include your name, a brief description of each photo, and a sentence about what it represents.

TERMS AND CONDITIONS

1. Eligibility: Sunridge Mall Clinic Photo Contest is open to all patients, staff, and community members who are over 18 years of age and reside in Canada. 

2. Submission Requirements: Participants may submit up to 3 original colour or black and white photographs using this form. Images must be high-resolution and suitable for public display purposes. All skill levels are welcome. 

3. Content Guidelines: Photos must align with contest themes (Diversity, See Yourself, Community in Space). Submissions should be appropriate for a healthcare setting and respectful of all individuals. Photos must be in their original state and cannot be altered in any way, including but not limited to removing, adding, reversing, or distorting subjects within the frame. 

4. Rights and Usage: By submitting this form, you confirm that you are the owner of the images and grant Mosaic Primary Care Network the right to retain, display and alter any photos submitted. No third party may own or control any materials the photo contains, and the photo must not infringe upon the trademark, copyright, moral rights, intellectual rights, or rights of privacy of any entity or person.

5. Privacy: You must ensure that you have permission from any individuals prominently featured in your photos. Do not include personal identifying information of others without consent. If a minor is present in any images submitted, a legal guardian must consent for them by submitting the form.

6. Deadline: All submissions must be received by December 15th, 2025. Late entries will not be considered.

7. Selection Process: Winners will be selected via a committee comprised of Mosaic staff and community partners.

8. Display: Selected photos may be displayed in the Sunridge Mall Clinic and used in related materials and shared on our Mosaic Primary Care Network's social channels.

9. Liability: Participants submit photos at their own risk. Mosaic Primary Care Network is not liable for lost, damaged, or misdirected submissions. Violators of the terms and conditions will be removed from the contest and banned from entering future contests hosted by Mosaic Primary Care Network.
