Please complete the form below to register your team for our Thursday October 15th fundraiser. Please remember that there is a total of 6 players per team including the captain. We have early registration prices August 30th. In order to reserve a bay for your team this completed form and payment must of $600 must be received . Bays will be reserved on a first come, first serve basis. The fee for individuals registering has been reduced to $100 per person. For those that do not wish to golf but would like to come drink, eat and "bogey" with us, the spectator fee is $50 per person.
