Accident & Injury

I/we realise that participation in dance classes and activities could involve possible personal injury.

Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Fierce Dance Factor. I/we assume all liability for participation in online classes.

I/we agree to release and hold harmless Fierce Dance Factor including its owners, teachers, dancers, staff members and facilities used from any cause of action, claims or demands now and in the future.

I/we will not hold Fierce Dance Factor liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes.

Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we cause to the facilities used by Fierce Dance Factor.

Transportation

I/we understand that parents are responsible for transportation to and from any and all events. Fierce Dance Factor is not liable for students who rely on rides from other dancers, parents and/or teachers. I/we recognize that by entering a teacher’s vehicle, as with any activity requiring motor vehicle transportation, my child may risk personal injury or permanent loss.

I/we hereby attest/verify that I/we have been advised of the potential risks and that I/we assume any expenses that may be incurred in the event of an accident, illness, other incapacities, or loss, and do not hold Fierce Dance Factor or its teachers/staff members liable for any of these charges.

Payment, Refunds & Late Fees

I/we understand that I/we are responsible for any charges and payments that apply to my account. I/we acknowledge that late payment fees will occur on any payments that are received more than two weeks after the payment due date. Fierce Dance Factor has the right to charge late payment fees and dismiss dancers who have not made payments in a timely manner, with no refunds on previous payments.

I/we acknowledge that a late payment charge of $30.00 will be charged to all recreational costume fees when overdue payment exceeds two weeks.

I/we acknowledge that class fees will accrue a $15.00 late fee, every 2 weeks exceeding 2 weeks of the payment due date, if not otherwise discussed and arranged with the owners of Fierce Dance Factor.

I/we acknowledge that an NSF fee of $20.00 will be charged to my account for any and all returned cheques.

I/we understand that the yearly registration fee is non-refundable. I/we acknowledge that costume deposits and costume fees are not eligible for a full or partial refund.

I/we understand that I/we will not be eligible for any refunds to any class payments made for the first semester. I/we understand that eligibility for a 50% refund on payments made for the second semester is available up until November 1, 2025.

I/we understand that clients dropping classes due to injury or illness will be eligible for a pro-rated credit to their account, with a supporting doctor's note.

PLEASE NOTE: All refunds will be subject to a processing fee.
