• Lift It LLC Request for Credit Application

  • Additional Company Information

    (Required for Credit Approval)
  • By signing below, the Applicant acknowledges and agrees to the following terms and conditions governing credit, and acknowledges they have authority and consent to make credit purchase decisions within their company. Purchases are made under a Net 30 payment arrangement with "Lift It Crane and Rigging." All invoices are due Net 30 days from the invoice date, unless otherwise agreed in writing. Payments physically mailed in the form of cash or check must be received by Day 30, not simply postmarked or mailed by that date. A one-time late fee of up to 20% of the total invoice amount may be applied, at the discretion of "Lift It Crane and Rigging," to any invoice not paid within terms. This is not a finance or interest charge, but a non-negotiable office processing fee incurred due to administrative time and effort associated with managing delinquent accounts. In addition to the above, a finance charge of 1.5% per month (18% annually) may be assessed on all past due balances until paid in full. Applicant agrees to pay all reasonable collection costs, attorney fees, court costs, and any other expenses incurred by "Lift It Crane and Rigging" as a result of non-payment or default. I consent to a UCC filing on any and all assets in the event my account goes 30 days past due. "Lift It Crane and Rigging" reserves the right to revoke or modify credit terms at any time, with or without notice, at its sole discretion. In this event, payments will be due by the end of the original term prior to revocation. Payments will be applied first to any office processing fees or finance charges, then to the oldest outstanding invoices, unless otherwise specified in writing. Any invoice disputes must be reported in writing within 7 calendar days of the invoice date. Failure to notify within this timeframe constitutes acceptance of the invoice as accurate and payable in full. This agreement shall be governed by and construed in accordance with the laws of the State of Indiana. All legal proceedings shall be filed in the appropriate jurisdiction within the state.
