    INSTRUCTIONS

    The prospective tenant of the apartment should complete the annexed application and attach the following items to the application:

    1. Complete applications in its entirety (sign and date where indicated If all the information is not provided, it will delay the processing of the application.
    2. Copy of your last four (4) pay stubs.
    3. Complete tax returns and W-2 forms for the past two (2) years, including all schedules (Federal and State).
    4. Credit check signed by each applicant.
    5. Statement of Financial Condition (form enclosed), copies of records, statements or agreements to support this statement, copies of the last three (3) bank statements
    6. Verification of residence letter from landlord/management as to the length of time of your residence and payment history
    7. Three (3) letters of reference (cannot be from relatives) for each occupant 18 years of age or older
    8. Photo Identification for each individual 18 years of age or older

    1. Management will utilize my/our application to obtain background information regarding the application.
    2. Management may require additional information.
    3. In no event will the Corporation or its agents be responsible for any liabilities or expenses incurred by any applicant whose application is disapproved.
    4. Falsification of any of the following information, or omission of material information herein, may result, without limitation, in revocation of the approval.
    5. The undersigned hereby authorizes management to contact any of the employers, banks, landlords, educational institutions, references, etc., described herein to elicit information bearing upon this application.
    6. The undersigned confirms the accuracy of all the information contained herein.
    7. The undersigned acknowledges that, if this Application is accepted, the undersigned will not, without prior written consent:
      • Permit non-family members to reside in the Apartment for more than one month.
      • Use the Apartment for purposes other than residential purposes;
      • Violate any provision of the Lease.

  • Our policy is to screen all applicants for the purpose of collecting relevant information, including but not limited to a credit profile search, interview with you, your friends, neighbors, present and past employers, and other references.

    I/we have read the above agreement and agree to its terms. I/we do hereby permit the management, its attorneys, managing agents, etc. to conduct a credit search and obtain information from others regarding my/our background. I/we have received a copy of this document and credit profile search, and I authorize same.

  • The application information provided by you may be used to obtain a tenant screening report; the name and address of the consumer reporting agency or agencies which will be used to obtain such report is/are: Trans Union, 2 Baldwin Place, P.O. Box 2000, Chester, PA, 19022; Equifax, P.O. Box 740241, Atlanta, GA, 30374, Experian, 475 Anton Blvd., Bldg. D, Costa Mesa, CA, 92626. Pursuant to federal and state law:

    (1) If we take adverse action against you on the basis of information contained in a tenant screening report, we must notify you that such action was taken and supply you with the name and address of the consumer reporting agency that provided the tenant screening report on the basis of which such action was taken;

    (2) If any adverse action is taken against you based on information contained in a tenant screening report, you have the right to inspect and receive a free copy of that report by contacting the consumer reporting agency;

    (3) Every tenant or prospective tenant is entitled to one free tenant screening report from each national consumer reporting agency annually, in addition to a credit report that should be obtained from www.annualcreditreport.com and

    (4) Every tenant or prospective tenant may dispute inaccurate or incorrect information contained in a tenant screening report directly with the consumer reporting agency.
