By signing below, I/we authorize Capital Hero to: (1) Obtain and review personal and business credit reports from credit agencies; (2) Verify all information through banks, creditors, and other parties; (3) Share application with lenders and funding sources; (4)Contact applicant via phone, email, text regarding application and future opportunities; (5)Review bank statements and financial records for underwriting.

I/we certify all information is true and accurate. False information may result in denial or legal action. Submission does not guarantee approval. Capital Hero acts as a broker presenting applications to various lenders. This authorization remains effective until revoked in writing.
