PARENTAL CONSENT

I, the undersigned parent/legal guardian of _______, born on _______, and currently enrolled at Dasmarinas Integrated High School as _______ Section _______,hereby give my full and informed consent for my child to apply and, if selected, to serve as a staff member of Ang Silayan.

I understand that being a staff member of Ang Silayan may involve responsibilities such as contributing to publications, attending meetings and allotting additional time in school, participating in organizational activities, and representing Ang Silayan in certain capacities.

I have been informed of the nature and purpose of Ang Silayan, including its adherence to its Constitution and Bylaws. I am aware that as a staff member, my child will be expected to uphold the principles and guidelines of the organization.

I confirm that my child has expressed their willingness to serve as a staff member of Ang Silayan, and I believe that this experience will be beneficial for their personal, intellectual, social, and leadership development.

I hereby release Ang Silayan, its officers, advisors, and members from any liability for any unforeseen incidents that may occur during my child's participation as a staff member, provided that such incidents are not a direct result of gross negligence or willful misconduct on the part of Ang Silayan.
