Authorization, Release, and Waiver (please read carefully)
In exchange for being allowed to participate in this event or trip, I acknowledge the following:
Medical Authorization & Emergency Care:
I authorize KBI staff or trip leaders to seek medical treatment for me if I am injured, become ill, or encounter an emergency, whether in the U.S. or abroad. I understand basic health information may be shared with medical professionals as needed for my safety, and it will be handled according to privacy standards.
Assumption of Risk & Liability Release: I understand participation may involve risks such as travel, physical activity, communicable diseases (including COVID-19), and unpredictable circumstances. I freely accept these risks and release Kingdom Building International, its staff, agents, volunteers, and board members from liability for injury, illness, or damages—except in cases of gross negligence or willful misconduct. I agree this waiver is legally binding to the fullest extent permitted under Texas law.
Insurance Responsibility: I acknowledge KBI recommends and expects me to have personal health and travel insurance (including for overseas trips and cruises). I accept full responsibility for any costs not covered by my insurance, including medical care, evacuation, or related expenses.
Photo/Media Consent: I grant KBI permission to take and use my photos, audio, and video recordings for promotional and ministry purposes—on social media, in publications, and on their website—without compensation. This consent lasts indefinitely unless revoked in writing.
Minor Participant Clause (if under 18): If I am under 18, my parent/legal guardian must sign below, confirming they’ve read this waiver with me and agree to all terms.
