I, the undersigned, understand and agree to all service policies, liability terms, and payment conditions set forth by Mella Salon. I acknowledge that service pricing may vary based on my selected stylist, hair length, texture, thickness, and overall hair condition. I understand that chemical treatments and hair extension services may affect hair differently and that multiple sessions may be required to achieve my desired results. I am aware that hair extensions—whether tape-in, bonded, sewn, or clip-in—can cause tension, discomfort, damage, or breakage to the natural hair and scalp depending on hair type, care routine, and maintenance. I agree to follow all aftercare instructions provided by my stylist and understand that failure to do so may affect the results and longevity of the service. I release Mella Salon and its staff from any liability for undesired results, allergic reactions, irritation, or damage to my hair or scalp related to any salon services, including hair extensions. I confirm that I have disclosed all relevant medical conditions, allergies, sensitivities, or medications that may impact my services. I understand that a credit card must be stored on file to hold my appointment, and failure to do so will result in automatic cancellation. I acknowledge that refunds are not permitted under any circumstances. I agree to provide at least 72 hours’ notice for cancellations and understand that late cancellations or no-shows will result in a 50% charge, and I may be restricted from future bookings until fees are paid. I accept that a 15-minute grace period is allowed for tardiness, after which my appointment may be forfeited. I am aware that Mella Salon charges a 3% fee and accepts all major credit cards, and does not accept checks, Zelle, Cash App, or Venmo. By signing below, I confirm I have read, understood, and voluntarily consent to these policies and terms, and release Mella Salon and its team from any and all liability related to the services I receive.
