Release Information
By submitting this form, you allow Thomaston First Methodist Church to seek whatever medical treatment is deemed necessary, and release the church and it's staff or volunteers of any liability. In the case of medical emergency, you take responsoibility for medical care and cost of any care provided to your student. You give consent for your student to be involved in the youth ministry of Thomaston First Methodist Church including on-site, off-site events, and transportation to and from said events. And you understand that media of your child may be used in digital or physical materials for the church and/or youth ministry.
