Release of Liability: Out Hustle Athletics LLC – Waiver, Release, and Parental Consent Form
I hereby waive, release, and discharge any and all claims for damages for personal injury, death, or property damage that I may have now or that may hereafter accrue to me as a result of my (or my child’s) participation in basketball practices, games, camps, or competitions organized by Out Hustle Athletics LLC. I acknowledge that basketball & athletics is a physical, contact sport, and that players of all skill levels may be injured while participating. As a parent/guardian, I understand these risks and voluntarily consent to allow my child to participate in all Out Hustle Athletics LLC activities. I affirm that my child is physically able to participate and that I carry appropriate medical insurance coverage. I agree to hold harmless and indemnify Out Hustle Athletics LLC, its owners, coaches, volunteers, and any facilities used for these activities from any and all liability for injury, death, or property damage arising out of participation. This release is intended to discharge in advance Out Hustle Athletics LLC, its staff, volunteers, and any facility used, from any and all claims or liabilities arising out of or connected in any way with participation in its activities, even if such claims arise out of negligence. Knowing the risks, I voluntarily assume all responsibility for loss, damage, or injury that may occur and agree to indemnify and hold harmless Out Hustle Athletics LLC from any costs or expenses related to such incidents.
Media Release:
I give permission to Out Hustle Athletics LLC, its coaches, and volunteers to photograph, record, and use my child’s name, voice, likeness, and image in photographs or audiovisual materials for lawful purposes, including instruction, advertising, the program website, social media, brochures, or publications.
Parental Consent (Required if participant is under 18):
I, the undersigned parent/guardian, give my consent for my child to participate in the above activities. I have read and fully understand the above waiver, release, and assumption of risk, and I hereby agree to its terms on behalf of my child. I further agree to indemnify and hold harmless the parties listed above from any liability arising from my child’s participation.
Important Disclosure:
I understand that Out Hustle Athletics LLC is an independent organization and has no affiliation with any middle or high school basketball programs in Westchester County, NY. Participation in this program does not guarantee placement on any school team.
