TRAINING AGREEMENT & CONTRACT
By submitting this document, I am voluntarily participating in firearms or other related self-protection training. I do hereby release the instructor(s) from any injury caused by or resulting from my actions or the actions of other students. I am aware that my participation in any class does not guarantee successful completion. I am also aware that the Instructor may remove me from the class for repeated unsafe actions that place others in danger. I agree if requested to pay a non-refundable deposit of $25 for any training and the balance is due in cash on the day of training. I agree and allow the Instructor to taking still and/or video images of me for his records and that those records will not be published for public viewing without my permission.
