TRAINING AGREEMENT & CONTRACT
By submitting this document.
I am voluntarily participating in firearms or other related self-protection training. I do hereby release the instructor(s) from any injury caused by or resulting from my actions or the actions of other students. I am aware that my participation does not guarantee successful completion. I am also aware that the Instructor may disqualify me for repeated unsafe actions that place others in danger. I agree when requested to submit my state issued identification and to pay a non-refundable deposit to complete my registration. I understand that the Instructor may conduct a criminal background investigation on me and will also take still and/or video images of me for his records and that those records will not be published for public viewing. I understand that this registration is also a club membership application that is required to attend any training. Club membership is free after the first training course and after approval of the Training Team. All class documents and information is private and is only available to the student and the club training team.
