By signing this form or registering through SkillsConnectAcademy.com or Registration Form, I acknowledge and agree to the following:
Assumption of Risk: I understand that participation in workshops, classes, and activities involves physical movement and creative tools, and I voluntarily assume all associated risks.
Medical Authorization: In the event of an emergency, I authorize Skills Connect Academy and its staff to seek necessary medical treatment for my child and understand that I am responsible for any costs incurred.
Photo/Video Release: I grant permission for Skills Connect Academy to take photographs and videos of my child during workshops for promotional purposes unless I submit a written request to opt out.
Release of Liability: I hereby release and hold harmless Skills Connect Academy, its staff, volunteers, and affiliates from any and all claims, liabilities, or damages arising from participation in any workshop, field trip, or event.
Behavior Expectations: I understand that respectful behavior is required. Disruptive or unsafe conduct may result in dismissal without a refund.
