In order to ensure the safety of our students, Berkeley Preparatory Academy requires that any student walking home have a signed permission slip in the office.
The procedure for this process is the following:
- Parent/Guardian Permission (by signing and returning this form, permission is granted).
- Each day, students who are walking will be escorted to the cafeteria at 3:00 during the start of Dismissal.
- They will check-in with the staff member on duty using PikMyKid to ensure that they are permitted to walk.
- If the student is in K-2, they will need an authorized chaperone, which will meet them in front of the cafeteria.
- Students will obey the crossing guard and be directed off of the school campus.
I understand that in granting this permission, my child’s teacher is authorized to release my child from the school with student walkers. I also understand that my child must leave school property at dismissal time and will not be allowed to linger on school grounds. If my plans change and my child needs to attend an after school program or will be picked up by carline, I will update PikMyKid and/or contact the school office with instructions for my child before 2:00.
This Walkers Dismissal Permission Form is understood by the student and the parent/guardian of the walker represented on this form to be a release of liability of Berkeley Preparatory Academy and its staff from any and all activities/incidents that occur as a result of walking home.
I give permission for my child to walk home from school each day unless I otherwise inform the office. I understand that students who are walking home are expected to walk directly home. If they are in Second Grade or younger, I understand that they must be accompanied by a parent or older sibling.
