I hereby agree to indemnify, defend and hold the Sam Hill Warehouse and StarStruck Event Planning, harmless in any action, claim or lawsuit arising out of or related to the preparation of any food and beverage brought into the Sam Hill Warehouse for the Prescott Wedding Expo taking place on March 1, 2026.
I acknowledge that NO frying or open flame cooking is allowed.
This form must be accompanied by Certficiate(s) of Insurance naming the Sam Hill Warehouse and StarStruck Event Planning with the below insurance requirements.
Please note that a cleaning fee or damage fee of up to $500.00 may be assessed to any food vendor that requires floor cleaning or floor repair after booth breakdown.
Please sign this document and provide the appropriate Certificate(s) of Insurance at least 2 weeks prior to the Event.
All Sponsors/ Exhibitors providing food or beverage samples must follow the Yavapai County Health Department rules/regulations and submit the proper and current licensing for special events to the department directly.
