REGISTRATION & FEES
Registration is based on a first-come, first-served basis. Classes are non-refundable or transferable.
You may sign up for more than one program. Each program is 1 class per week for 5 weeks.
$140.00 per 5 week session
Once registration form is received, an invoice will be sent for the available classes.
WAIVER
I hereby authorize my child's participation in the SMS Infant Toddler Program. I know of no issues which may affect my child's ability to participate safely in this program.
I agree that any pictures or video taken by SMS or by persons authorized by the school may be reproduced and used without liability for school purposes, unless I indicate otherwise in the coment box below.
