Race Release Form

In consideration of the acceptance of my entry, I do hereby, for myself, my successors, my heirs, executors and administrators, and if I am signing on behalf of a minor, waive, release and forever discharge any and all rights and claims for damages which I may have or which may hereafter accrue to me against the Pateros Jamboree Jog, run personnel, sponsors, and all municipal agencies whose property is used, and all officers, directors, trustees, employees, volunteers, and agents of the foregoing entities for any injuries or damages including death that I may suffer as a result of my participation in the Pateros Jamboree Jog. I attest and verify that I am physically fit and have sufficiently trained for the completion of this event. I have read the entry information and certify my compliance by my signature. I also understand my entry fee is nonrefundable unless the event is canceled.
