1. Fees
Fees shall be paid by cash, cheque, standing order or direct debit, made payable to Made-Well CIC;
The current fees are £70.70 per day, half day £35.35 and are payable every 4 weeks. The payment due date is stated on your invoice. One to one support is £24.08 an hour.
Transport costs within the local area are £15.00 per return trip.
Full day times are 10am until 3pm, half day times are 10am until 1pm or 12pm until 3pm
We reserve the right to adjust fees as required and will give you at least 4 weeks' notice of any changes;
Payment for any outings is additional to the fees and will be charged separately;
Payment for any individual 1:1 support needed from Made-Well staff or contractors will be agreed with you and charged separately to attendance fees;
If you have any difficulty in paying your fees we can arrange a payment plan. If we cannot agree a payment plan or if this plan is not followed, then we may refuse attendance and take action to recover outstanding fees;
If fees are overdue for more than 14 days and there is no payment plan agreed, then only 1 week's notice will be given to terminate the contract;
Overdue payments will be followed up by letter. We reserve the right to make an additional charge of £5 for administration costs per letter.
2. Absence
Made-well will charge for all booked attendances unless 4 week's notice has been given of a change due to sickness, holiday or other reasons;
Fees paid for sessions not taken due to sickness or holidays (except for pre- arranged holidays as set out below) and other reasons are not refundable;
There may be exceptions for short term absence due to family bereavement or major crisis in your life;
You (or your representative, family or carer) must make Made-Well aware as soon as possible so that your absence can be noted, and fees adjusted if appropriate;
We will not charge participants with long-standing health issues for the time they need to regain good health. In the case of absence due to ill-health that is expected to last longer than 4 weeks, please contact Made-Well to discuss suspending this agreement until an agreed return date.
3. Operating Hours & Times
Made-Well is open 7 days a week from 9am to 4pm;
Made-Well is open to the public at other times, including weekends and people are welcome to be part of these activities but this does not form part of this agreement and no responsibility for support will be taken by Made-Well;
Made-Well closes for the Christmas period. A notice of closing times will be posted on our notice board and we will write to you;
Made-Well reserves the right to close at other times if required / in an emergency and will notify you if this is necessary.
4. Termination of this agreement
You need to give us at least 4 week's notice to cancel this agreement. You are liable to pay fees for booked attendances in the 4 weeks from the date you give notice;
Made-Well can cancel this agreement at any time by giving you 4 week's notice.
I have read and understood the conditions set out above and agree to comply with them.
