Waiver & Consent Acknowledgment
By signing below, I acknowledge and agree to the following:
Service Risks: I understand that hair and skin services may carry some risks, including possible allergic reactions, skin irritation, hair breakage, or results that differ from my expectations. My stylist will always take reasonable care, and I release them from liability for any reaction or outcome that may occur.
Health & Medical Disclosure: I have shared all known allergies, sensitivities, and medical conditions that may affect my service. I understand that not providing this information could increase the chance of an undesired result or reaction.
Results Disclaimer: I understand that every guest’s hair is unique and results may vary depending on my hair’s current condition, history, and maintenance. Some services—such as blonding, vivid colors, corrective color, perms, or smoothing treatments—may take multiple sessions and consistent professional and at-home care. No specific result is guaranteed.
Adjustments: If I am unhappy with my service, I agree to let my stylist know within 7 days of my appointment. Adjustments will be considered at the stylist’s discretion. After this time, any corrections or new services will be treated as a separate appointment with normal service fees.
No Refunds Policy:I understand that all services are non-refundable. Once a service has been performed, payment is final.
Late Arrival, Cancellation & No-Show: Life happens, and I understand that schedules sometimes change. If I arrive more than 15 minutes late, my appointment may need to be rescheduled and a cancellation fee may apply. Cancellations made with less than 24 hours’ notice will result in a 25% cancellation fee, and missed appointments (no-shows) will be charged 50% of the scheduled service. A deposit may be required to secure future appointments.
Agreement:I confirm that the information I have provided is accurate and true. I understand that hair results depend on starting color, previous chemical services, and hair condition. My stylist will discuss realistic expectations during my consultation. I have read and understood the above policies, and by signing this form, I voluntarily agree to proceed with the requested services and accept responsibility for the outcome.
