I Understand & Agree* This field is required.Wedding Day Dog Attendant services include: Initial consultation A 30-minute dog visit before the wedding. The visit is to be scheduled at least two weeks before the wedding. Depending on the package selected either three or five hours of care on your wedding day, including my travel time. Included is travel up to 100 miles round-trip between 75071 for dog pick up, the wedding venue, and returning the dog to the designated place. Coordination and planning of your dog's activities and timeline with the wedding planner, photographer, couple, or family for seamless pet involvement. Snacks, leash walk, poo pick-up, and disposal before, during, and after the ceremony to ensure the dog's comfort. Dressing your dog with attire & accessories for the wedding - if agreed to and purchased before the wedding, and as long as the dog remains comfortable Individualized and professional dog care and supervision. Personalized assistance with your photographer while the dog is getting their picture taken. A non-refundable deposit of 50% of the total package cost is required to secure your reservation. This deposit serves as a credit towards the full balance. The remaining balance is due sixty (60) days before the wedding. If the wedding is scheduled less than 60 days in advance, full payment is due. If cancellation is made less than sixty (60) days before the wedding, no refunds will be issued. If greater than sixty days notice is given you will receive any amounts paid in excess of the 50% deposit minus administrative fees ($150) will be returned. I understand and agree that it is solely my responsibility for any harm or damages caused by my dog while it is in the care of Lynn Molnar (The Dog Attendant). The Dog Attendant its owners, employees, representatives, or any person affiliated with the company shall not be held liable for any health problems, illness, or injury, including death, that may result from the action or inaction of the Dog Attendant owner, employee, or representative. I understand and agree that in the event of inclement weather, natural disaster, or act of war, The Dog Attendant is entrusted to use its best judgment in caring for the dog(s). I understand that the Dog Attendant does not accept responsibility or liability for my dog after it has been returned to the designated drop-off location. The Dog Attendant will provide photos and text message(s) indicating the dog has been securely dropped off.

I understand, waive, and relinquish any claims against The Dog Attendant, Lynn Molnar and its employees or representatives. I agree to notify The Dog Attendant of any concerns related to agreed-upon services within 24 hours of the dog's return home or to the designated drop-off location. Any matter not brought to us within this time frame will not be considered. I have provided the name and phone number of someone who is authorized to handle any major problems that may arise in my absence. If the contact person cannot be reached promptly, I agree that the Dog Attendant may resolve emergencies at its sole discretion, and if costs incurred, I will reimburse The Dog Attendant for any charges, including veterinarian costs, and The Dog Attendant time costs at $245 per hour. I understand that if my dog exhibits aggressive behavior of any kind, at any time, the Dog Attendant reserves the right to refuse or cancel service. No refunds will be issued. The owner will be liable for the medical and veterinarian expenses and/or damages that are the result of a dog bite or injury due to your dog's aggression. I understand that I will provide the Dog Attendant with proof of current rabies vaccinations for all my dogs that will be in the wedding. The Dog Attendant has a strict rule that the dog must remain on a leash for the entire time that we are caring for your dog. Only written modifications to this policy prior to the wedding will be binding. If someone at the wedding requests that the dog be off-leash or be handed the leash, The Dog Attendant will decline. It is the sole responsibility of the Dog Attendant to care for your dog(s), and we will not hand off the leash to someone else at any time. I understand that it is my responsibility to inform my wedding guests that they can interact with my dog, but under no circumstances will anyone other than the Dog Attendant be holding the leash. I consent to photographs or recordings of my dog by the Dog Attendant for promotional purposes without any compensation to me. I release all rights and privileges to said images or recordings.
