This form is intended for students in grades 6 through 12 who participate in school-sponsored extracurricular activities, including athletics and performing arts. Sixth-grade students may participate in the Middle School Play, but are not eligible for interscholastic sports until grade 7. These programs play an important role in promoting student engagement, leadership, and school spirit. To help offset the costs associated with offering these opportunities, a student activity fee is collected annually.
Activity Fee Structure
Standard Rate
Base Fee: $25 per student
Additional Fee: $25 for each extracurricular activity or sport
Reduced Lunch Program
Base Fee: $10 per student
Additional Fee: $10 for each extracurricular activity or sport
Free Lunch Program
No fees are required
Students who qualify for the Free Lunch Program do not need to complete this form. These students may check in at the Middle School or High School office to receive their activity sticker.
Payment and Admission
Once the base and activity fees are paid, an activity sticker will be issued and placed on the student’s school ID. This sticker grants free student admission to all regular-season home sporting events, as well as performances of the Middle School Play, High School Fall Play, and High School Spring Musical.
