An Endorsed Delegate is a registered member who is has been proposed by the club Chief Training Officer (CTO) to conduct inductions for club members. 

This form is to be used for the re-endorsement of Endorsed Delegates for the following awards only:

• • SSV and/or
  • Tractor and/or
  • Shallow Water Rescue Induction

Endorsed Delegates for the above awards must be re-endorsed annually and meet the pre-requesite Endorsed Delegate award criteria listed below each season.

Note: pre-requisite criteria are different for vehicle and SWR endorsed delegate awards.

Instructions on completing this Jotform:

1. The CTO is to list the proposed delegate’s full name and DOB.
2. Use the drop-down option and add the appropriate awards the member is to be re-endorsed for the season (more than one award per member can be selected).

SSV and Tractor pre-requisites for re-endorsement

• Hold a current relevant driver's licence
• Hold the Endorsed Delegate SSV and/or Tractor award

Shallow Water Rescue pre-requisites for re-endorsement

• Current Bronze Medallion or SRC award holder
• Hold the Endorsed Delegate Shallow Water Rescue Induction award
