The Trustees of Duchy Health Charity appreciate project completion reports to enable us to understand the impact of the work we are funding, and what progress you have made against your objectives.
To ensure that reporting is kept as simple as possible, we have designed this form which you may choose to use if you wish. There is no requirement on you to use this particular form. If you already have another format that you are using successfully then please feel free to use that instead.
