Order Form

If you are paying for your membership with check or cash, you will need to bring your payment to the next booster club meeting.

Check Out Instructions (Please Read)

Thank you for your contribution to the Booster Club and Choir Progam. 

• To Submit – The order form will not be sent until the payment has been processed. Pay via PayPal or Credit Card to send the order. If the balance is $0, click on either button, it will submit and bypass payment.
• Checkout – The checkout process will ask for your shipping address. Make it the same as your billing address. 
• Receipt – A receipt will be automatically sent via e-mail to the Parent 1's e-mail address.
