Elevé Dance Theater Registration Form 2025-2026

Welcome to The Academy at Elevé Dance Theater!

Our 10-month program offers young dancers the opportunity to train in 3 to 5 dance styles each week—all included in one set tuition price. Throughout the season, students build strong technique, explore different genres, and grow in confidence.

The Academy year culminates in our end-of-season recital each June, where dancers showcase their hard work and artistry. Plus, students enjoy performance opportunities throughout the community—sharing their talents and spreading the joy of dance beyond the studio!

At The Academy, students are thoughtfully placed into classes based on both age and skill level to ensure every dancer is challenged, supported, and able to thrive. Our leveled approach allows dancers to grow at the right pace while receiving personalized instruction in a nurturing environment.

Pre-Dance Academy

Pre-Dance I ( ages 3-4) - $100 per month

Pre-Dance II (ages 5-6) $125 per month

The Academy

$150 per month (3-5 classes per week)

Monthly tuition is due on 1st of every month. After the 5th of each month, payments will be considered late and a $35 late fee will be assessed.

Dancers will not be able to participate in class if at least on month of unpaid tuition

I ___________ understand and accept the financial requirements set forth by the Elevé Dance Theater, Inc. I also understand that any dancer that fails to satisfy all financial commitments may result in the exclusion of Elevé Dance Theater's final performance, tentative concert date June 27, 2025.

By signing this financial obligation form, I agree to submit payments in the full amount on the dates provided above. I understand and agree that all payments are non-refundable (personal checks will not be accepted).

I understand and accept that membership in Eleve Dance Theater Academy and/or Youth Ensemble means that membership in or at any other personal dance company or team is forbidden. *This does not include school or church related organizations.

ALL DANCERS MUST BE PICKED UP ON TIME. ELEVÉ DANCE THEATER WILL NOT BE RESPONSIBLE FOR DANCERS LEFT AFTER CLASS OR REHEARSAL IS OVER.

If for whatever reason the student listed above decides to discontinue any or all classes, I will complete and sign a withdrawal form and pay the wihdrawal fee.

This form must be completed during office hours with the office staff or director. Withdrawal forms may not be completed with teachers or over the phone/text.

Withdrawal forms must be completed more than 14 days prior to the beginning of the upcoming pay period. I understand I will still be completely responsible and give Elevé Dance Theater my permission to charge my bank account for the upcoming pay period’s entire tuition in the event I do not complete a withdrawal form more than 14 days prior to the start of the new pay period.

Liability Waiver/ Photography Release

I understand and acknowledge that there is a risk of injury inherent in dance activities and that personal injury or damage to property may result during participation in dance and related activities. I represent that ____________________________(child) is physically able to safely participate in dance and related activities. I agree to assume all risks associated with my child’s participation in dance instruction, rehearsal, performance and related activities. In consideration of receiving dance instruction at Elevé Dance Theater, I hereby waive, release and discharge all present and future claims and liabilities of any kind, whether for bodily injury, property damage, or other loss, arising out of my child’s participation in dance and related activities, including but not limited to dance instruction, rehearsals, and performances, whether conducted on or off Elevé Dance Theater premises.I also acknowledge that I am responsible for delivering my child to the studio and picking her/him up and that the studio is not responsible for a child that leaves the premises.

If parent or Emergency Contact cannot be reached in case of an emergency, consent is given for my child to receive medical or surgical care as recommended by the physician or hospital.

I have read this release, understand it, and hereby agree to its terms. I also consent to photography and recording of my child for its usage in promotional and public relations activities. All recordings are property of Elevé Dance Theater.

I have received the Elevé Dance Theater Registration Form with the 2024-2025 Studio Calendar and agree to the terms regarding all related program fees and deadlines. I understand and agree to the withdrawal policy of a minimum of a 14 day written notification form as well as any changes to the dancer’s class schedule.
