     By submitting this application, I confirm the information provided is accurate to the best of my knowledge. I understand this is a screening form and not a contract. If approved, I will be contacted with next steps to complete the Lease-On Agreement.

     

    Driver Record & Background Screening Authorization
    I hereby authorize Pullcision Dispatching and/or Sean Hall Hauling LLC, and their designated agents, to obtain and review my Motor Vehicle Record (MVR) and any other relevant background information for the purpose of determining my eligibility to operate under their authority, be leased on, or provide transportation services.

    I understand that this information may be obtained from state motor vehicle agencies and/or third-party consumer reporting agencies in accordance with the Fair Credit Reporting Act (FCRA) and applicable regulations.

    I certify that all information I provide is true and correct to the best of my knowledge. I understand that I may request a copy of my MVR or report at any time.

    I acknowledge that this authorization is ongoing for the duration of my relationship with the company, unless revoked in writing.

     

    This application is administered by Pullcision Dispatching LLC on behalf of the motor carrier identified below. Pullcision Dispatching LLC provides authority management, recruiting, onboarding, compliance, and administrative support services for independent motor carriers. Submission of this application does not constitute an application for employment or independent contractor services with Pullcision Dispatching LLC. Any driving position or independent contractor relationship will be solely with the motor carrier listed below, if approved.
