    Trans Affirm is a grassroots, trans-led nonprofit committed to increasing access to gender-affirming care across Idaho, especially in rural areas where resources are scarce. We created this HRT Access Grant to support trans and gender-diverse adults (18+) who are low-income or have lost access to HRT due to Medicaid cuts or other financial barriers.

    Due to limited funding, individuals may be awarded only one grant.

    For this first round of HRT grants, all awards will be coordinated through a single provider to help us keep the process streamlined and accessible as we launch the program. At this time, grant recipients will receive care through Dr. Jessica Rolynn. It covers one year of HRT through her clinic.

    Dr. Rolynn offers both telehealth services and in-person appointments for those able to travel to Pocatello. She practices at Prism Family Medicine and has been a consistent and trusted supporter of Trans Affirm and the broader local community. As a bisexual provider, she brings both professional and queer expertise and lived understanding to her care, and she also leads TAG (Trans Affirming Group), a peer support group based in Pocatello.

    While we hope to expand options in the future and allow recipients to choose their own providers, starting with one provider allows us to build a strong, manageable foundation for this program and ensure participants receive affirming, reliable care.

    Applicants should be prepared to complete and sign a Release of Information (ROI) form as part of the process. This allows for necessary coordination between Trans Affirm, the provider, and any required services to ensure care can be delivered efficiently.

    To be eligible, applicants must have completed an annual exam within the past 1-6 months (or work to receive one before starting HRT), as well as a follow-up visit within the last 6 months that includes updated vitals. These requirements are in place to help ensure medical readiness and safety when beginning or continuing HRT. Grants will be rewarded towards August 2026. 

    Once a grant is awarded, required lab work will be processed through Labcorp, unless Labcorp services are not available in the recipient’s area, in which case alternative arrangements will be made.

    We recognize that these requirements may feel like additional steps, but they are intended to support a smoother experience and ensure that recipients can access care as quickly and safely as possible. <3

    These are the current grants we are offering: 

    Continuing HRT Grant
    This grant is for individuals who are already on hormone replacement therapy and need financial assistance to maintain uninterrupted care. Eligible expenses include prescription costs, required lab work, follow-up appointments, and related medical fees. Applicants should be able to demonstrate current or recent HRT use and outline the estimated costs needed to continue care for the grant period. 

    New HRT Start Grant
    This grant supports individuals who have not yet begun HRT and need assistance to start. Covered costs may include initial provider visits, required lab work, prescriptions, and education around starting HRT safely. Applicants should describe their current stage in the process, any barriers they are facing, and whether they have identified or need help connecting with an affirming provider. This grant includes care navigation support to help recipients successfully initiate HRT.

    All applications are reviewed with an equity-based lens, prioritizing individuals who are low-income, uninsured or underinsured, and those living in rural or underserved areas of Idaho.

    This grant is open to:

    -Idaho residents age 18 or older (due to Idaho Law) who identify as Trans, non-binary, intersex, or anything under the trans umbrella and are not cis.
    -Individuals who are low-income OR lost access to HRT due to Medicaid no longer covering HRT
    -Those who are starting, restarting, or continuing HRT and need financial support for labs, appointments, or prescriptions due to financial difficulty 


    We are a small volunteer-run organization with limited funding. Because of this, we can’t guarantee the same number of grants or funding amounts each cycle. The number of recipients and the amount of each grant will depend entirely on how much funding we’re able to secure throughout the year.

    We know the need is high, and while we may not be able to support everyone each round, we’re doing everything we can to make care more accessible and affirming for our community.     Who can apply for this grant?
    This grant is for trans and gender-diverse adults (18+) who live in Idaho and are low-income or have lost access to HRT due to Medicaid changes or financial hardship. You must identify as under the trans umbrella and not be cisgender.


    What does the grant cover?
    It depends on our funding and your needs, but grants may cover:

    -HRT intake or restart appointments
    -Lab work
    -1–6 months of hormones (Or up to $4,000 worth) 
    -Possibly follow-up care (if budget allows)


    How much money will I receive?
    There is no fixed amount. Trans Affirm is a small nonprofit, and the amount of each grant may change based on available funding. On average, grants might range from $500 to $1,000, but that’s not guaranteed. It covers one year of HRT.


    How many grants will be given out?
    The number of grants we offer depends on our funding for the year. In some cycles, we may be able to support multiple people, and in others, we may only have enough for a few. For our first cycle, we are aiming for 5-6 humans to receive a grant from us!


    Will I get money directly?
    In most cases, no. We aim to pay providers directly for your care (appointments, labs, prescriptions). In rare or special cases, we may reimburse you with proper documentation. 


    Do I need documentation to prove income or HRT history?
    You can provide proof of income (such as SNAP, Medicaid denial, SSDI, etc.) or complete a self-attestation form. You do not need to be on HRT currently, but we may ask for basic info from a provider or caseworker confirming your need.


    Do I have to go to a specific doctor?
    For this grant cycle, yes. This grant is specific to Dr. Roylan, while others in the future may let you use the grant with any Idaho-based gender-affirming provider who can accept direct payment from us. If payment is too high, we may suggest other providers, or work with our max amount given instead, which may lessen the time covered by this grant. For the current cycle, anybody unable to travel to Pocatello for her clinic will be offered telehealth. 


    What if I don’t get selected? Can I apply again later?
    Yes! If you don’t receive a grant this cycle, you’re welcome to apply again in the future. We will keep your info on file for future opportunities (unless you ask us not to).

    We ask that if you have already received a grant, you wait until the next calendar year to reapply, and to request again only if you truly need more assistance.


    Is my information confidential?
    Yes. We take your privacy very seriously. Only authorized members of our team will see your application, and we will never share your name, gender history, or contact info without your permission.

    ⚠️ Please Read Before You Apply
    This grant is designed to help you start or restart HRT, and we know how difficult access can be — especially after losing coverage or facing financial hardship.

    That said, this funding is intended to be a stepping stone, not a long-term solution. It's meant to help you get the process going, but you should only apply if you have a realistic plan to continue HRT beyond the support we provide (even if that includes gaps or asking for help along the way).

    We completely understand that many people can't afford consistent care right now, and we're not expecting perfection. But it’s not medically safe to only take hormones for a few months and stop suddenly, especially without a plan in place. That can lead to health complications and emotional distress.

    If you are selected and receive support, and then choose to stop treatment mid-process (during the time we are actively covering appointments, labs, or medication), you will not be eligible for the remaining funds that were set aside for you.

    We say this not to gatekeep care, but to make sure these limited funds go to those who are ready and able to use them in a sustainable way. We care deeply about your health and safety, and we want this to be a meaningful, affirming part of your journey.

    If you are selected for this grant, Trans Affirm will only cover future costs, not reimbursements for past payments. Once you're approved, we’ll work with you to set up payment directly with your provider.

    We ask that you schedule your first appointment within 4 months of being approved. We understand that unexpected things can come up, and we’re happy to be flexible in cases of emergency or provider availability issues. However, we can’t hold funds due to delays like a lack of follow-through or motivation.

    Please only apply if you’re ready to take this next step in the coming months. This helps ensure that funding is going toward people actively seeking care and allows us to keep this support moving for others who need it.
