Instructions for Fellowship Application:

To apply for the St. Jude–VIVA Travelling Fellowship, applicants must complete the following steps in full. Incomplete or inaccurate submissions will result in immediate disqualification.

1. Complete the Fellowship Application Form
- Fill out all sections of the application form clearly and accurately.
- Ensure that all required fields are completed before submission.
- Incomplete forms will not be considered

2. Submit an Abstract or Case Presentation
- Medical applicants must submit an abstract relevant to their research or proposed project.
- Nursing applicants must submit a case presentation using the standard template provided.
- All submissions must follow the formatting and content guidelines provided by the SJVF organising team.
- Submissions will be reviewed by a panel of reviewers from VIVA, NUH, KKH, and St. Jude.

Only applicants who submit both a completed application and the required abstract/case presentation will be considered.

• Supportive Care, Outreach and others

Theme: “Integrating Palliative Support in Paediatric Oncology: A Case-Based Approach”

a. Declare that all the information provided by you (including any supporting documents) for the purposes of the Application is true and correct. In the event that any information provided by you for the purposes of the Application is subsequently found to be false, misleading or inaccurate, VIVA Foundation reserves the right to withdraw any approval granted in relation to your Application.

b. Consent to your personally identifiable data being collected, stored, and used by VIVA Foundation for the purposes of processing your Application, and/or for any other purpose relating to VIVA Foundation’s operations.

c. Agree to attend the St Jude-VIVA Forum on 5 - 8 March 2026 and the VIVA-Asia Regional Educational Day.

d. Agree to not directly or indirectly engage in any business that will harm VIVA's interest. Nor shall you solicit any staff, employee, partner, sponsor, donor for the benefit of himself/herself or a third party.

e. Understand and Agree that photographic or video recordings of me may be electronically displayed, including but not limited to via the Internet. There is no geographic limitation on where these materials may be distributed. I hereby release any and all claims against VIVA Foundation in utilising Materials consistent with this Application.
