I agree and understand that serious accidents can and do occur during dog training. competitions , and other dog related events and activities. I am assuming all risk liabilities and responsibilities and will hold harmless PSAK9, Clay’s Top Dog, Top Dog K9, their agents, partners, members, and employees from any and all liability arising from this dog-related event and any practice, training, or open field. I also agree to hold harmless any of the above mentioned from property damage, theft, death or personal injury to my dog or myself.
I agree to have complete control and responsibility over my dog and myself at all times. By submitting this registration form I acknowledge that I have read and understand the trial waiver and read and understand all PSA rules that govern trials.
There will be no refunds given to any hander that has been ejected from this trial for any reason. In the event a handler / team is unable to attend this event and their reserved spot in the trial is able to be filled by the trial secretary by another paying competitior.
In the event of any unintentional overbooking of the event a full refund will be issued to any registrant who did not receive one of the alloted slots.
In the event all slots are sold you may contact the hosting club or trial secretary and request to be placed on a waiting list. All registrations and waiting lists will be served first come - first served bases.
I agree to stay informed of any event updates and changes via the PSA website event listing, and the Facebook Event Listing for this particular Trial.
Entry forms submitted without payment are not offical. All payments must be received prior to any registration becoming official. I agree it is my responsibility to ensure that I have properly fllled out above registration form and will be submitting payment upon receipt of email notification or other notification of successful registration.
In addition, I agree and give consent to any representative of PSA K9, Clay’s Top Dog, Top Dog K9 to take pictures of the events and activities surrounding the events, and that said pictures shall be the sole and exclusive property of Clay’s Top Dog for any use or distribution at their sole discretion. Further I release any rights to any videos or photographs taken by Top Dog K9, Clay’s Top Dog or other photographs taken at this public event.

By signing below, I affirm that:
1. I have reviewed and understand the current PSA–AS Bylaws and Code of Conduct as published on the PSA-AS website as of the date of signature.
2. I agree to abide by all rules, regulations, and standards of conduct set forth in these documents.
3. I understand that compliance with the Bylaws and Code of Conduct is a condition of my membership and/or participation in any sanctioned PSA–AS event.
4. I acknowledge that any violation of these rules may result in disciplinary action, up to and including suspension, revocation of membership, or a lifetime ban, in accordance with the disciplinary procedures outlined in the PSA–AS Bylaws and Code of Conduct.
5. I understand that PSA–AS reserves the right to amend or update its Bylaws and Code of Conduct, and that it is my responsibility to remain informed about any such changes.
6. I understand that PSA–AS reserves the right to amend or update its Bylaws and Code of Conduct. I acknowledge it is my obligation to stay informed about any changes to these documents, which will be made available via the official PSA–AS website and/or the official PSA–AS Facebook group page.
