Mobile Amenity Credential Reactivation
Complete this form if your mobile amenity access credential has been deactivated or expired. Our staff will review submissions and contact you with a solution or to request more information within 24-48 hours (or on the next business day if submitted when the office is closed for a holiday or the weekend).
If you have previously completed a waiver, please DO NOT complete a new waiver unless prompted to by staff.
Reasons that access cards may be deactivated include but are not limited to the following reasons listed below:
- The resident no longer lives in the Heartland Community Association.
- The homeowner has rented out their house and has given access to their tenants.
- The resident violates facility use rules.
- Your lease has expired and the resident needs to submit an updated lease.
- The homeowner is delinquent on their HOA assessments or fines.
- Technical issues outside of the power of the Heartland management team's control.
Once your form is submitted you will receive one of two emails:
- If the staff does not need any more information, you will receive an email with a new credential link from Sentry Security Solutions.
- If the staff requires more information you will receive an email directly from one of the staff members at the Heartland management office.
