    Please list all moving traffic violations you’ve been convicted of within the last three (3) years. Do not include parking violations.
  • Pre-Employment Drug & Alcohol Screening (For CDL Positions Only – 49 CFR § 40.25(j))

  • You are required to upload documentation of successful completion of the Return-to-Duty process (per 49 CFR Part 40 Subpart O).
    If your pre-employment test with Dunn Transit is positive, you will be referred to a Substance Abuse Professional (SAP) in accordance with 49 CFR § 40.287.

    This information may include contact with former employers, third-party background check providers, and any substance abuse professionals (SAPs) involved in a Return-to-Duty process, if applicable.

    You have the right to:
    • Be notified before any adverse action is taken
    • Receive a copy of any reports
    • Dispute inaccurate information

    This authorization is valid throughout the hiring process and, if hired, during your employment.
