Important Event Information

All registered vendors must complete Hold Harmless and release of liability form below before receiving their confirmation packet and being allowed to participate in the Second Missionary Baptist Church Community Health Day.

Hold Harmless and Release of Liability Clause
In consideration for being allowed to participate in this voluntary Health Day event, I release from liability and waive my right to sue Second Missionary Baptist Church, their employees, officers, members and agents (collectively "SMBC") from any and all claims, including claims of SMBC's negligence, resulting in any physical injury, illness (including death) or economic loss I may suffer or which may result from my participation in this Health Day and any activities incidental to the event.*
I agree to hold the Second Missionary Baptist Church, their employees, officers, members and agents (collectively “SMBC”) harmless from any and all claims, loss or damage to my personal property, liabilities and costs, including attorney’s fees, as a result of my participation in this Health Day event including any activities incidental to this event. If SMBC incurs any of these types of expenses, I agree to reimburse SMBC.*
