Conduct Agreement

I acknowledge that all campers are expected to demonstrate respectful behavior toward staff, coaches, fellow campers, and the facility.

Any camper who exhibits repeated disruptive, disrespectful, or unsafe behavior may be sent home at the discretion of the event organizers.

I understand that if a camper is dismissed from camp due to misconduct, no refund will be issued.

I agree to the code of conduct and understand the refund policy.

Liability Waiver & Medical Consent

By checking "Yes," I confirm that I am the parent or legal guardian of the camper named above. I acknowledge that participation in the “Beyond the Game” Basketball Camp involves physical activity and assume all risks related to such participation. I hereby release and hold harmless Fox Sports Radio, PickUp USA Fitness, their staff, and affiliates from any liability, claims, or causes of action arising from any injuries sustained during camp. I also authorize camp staff to secure any medical treatment deemed necessary in case of emergency.
