  • This is the Easy Read application form for the Main Exhibition. It is for all kinds of art, including video.

    This is not the application form for the mini-books. 

     

    You should fill out this form using a desktop computer or laptop. It will be hard to read on your phone.

  • About You

  • A blank name badge. Text: what is your name?
  • An email symbol. Text: What is your email address? Please use the one you signed up to Neuk with.
  • A map and a house. Text: What is your address?
  • A booklet marked "About the artists". Text: Tell us a bit about you as an artist. This is called an "artist bio". Up to 100 words.
  • A woman thinking. Text: You could tell us about how or where you learned to make art; what you have done in the past; what you are interested; in something else

  • About Your Work
