By completing this form you confirm that the member is fit and healthy to take part in workshops AND public performances. However, please state information regarding you/your child’s health, mobility, behaviour and medication in order to help us meet his/her needs more fully whilst at NK. Or are there any special protocol/procedures we should be aware of in dealing with your child?
As part of our Terms and Conditions of Membership, we require all members to provide a Registered Chaperone representative. Anyone can register as a chaperone, a family member, friend etc. Please provide in the box below the name of the chaperone, together with their license expiry date, DBS number and the name of the authority who issued the license. If you currently do not have a representative with a licence, please put the name of the person below who will be making an application.*
Occasionally theatre sessions may be filmed or photographed for monitoring and general press and publicity purposes, please state if you give permission for this*

We are requesting one payment for NK Memberships to be paid monthly, preferably on the 1st or the 15th of the month. This should be one regular payment for the amount to cover the classes being attended. Please provide the bank reference associated the member, below.
