UNM FSL Chapter Event Form Registration 2025-2026

This event form is a general event form for chapters to submit their events. There will will be a drop down of what type of event it will be (e.g. chapter socials, philantrophy, recuitment events, informational, etc.) These events must be registered five (5) business days in advance.

Form Requirements

To complete this form, you will need:

• The time and date of your event;
• The names of the event monitors (one is required per 25 attendees);
• If your event is being held on campus, you must have a confirmed reservation from the venue.

You must input the email addresses of your Chapter's Risk Manager (or equivalent), President, and Advisor. It is your responsibility to ensure these are correct as incorrect emails will result in delays in the approval process.

The {pleaseSelect} chapter agrees to follow the UNM Fraternity & Sorority Life Event Management Policy. The chapter understands this policy does not supersede local and international fraternity or sorority risk management policies and procedures. It is up to the {pleaseSelect} chapter to research, comply, and complete any required events forms as requested by the fraternity or sorority General Headquarters. Failure to do so could mean a loss of chapter liability insurance coverage.

 

The undersigned declared that the statements set forth herein are true to the best of their knowledge and belief. The {pleaseSelect} chapter indemnifies and holds harmless The University of New Mexico, its officers, employees, and agents of and from any cost, loss, or expense incurred as a consequence of damage to property, personal injury, or death arising or alleged to have arisen out of the performance of this agreement, together with any legal and related expenses which may be incurred by my University of New Mexico in defending such claims unless such loss results solely from the negligent act or admission of The University of New Mexico.
